On 19 October 2026 a temporary banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013.

The Banning order prevents Ahmed Charchouh from:

1. Engaging in activities that a member of key personnel of a National Disability Insurance Scheme (NDIS) provider would undertake, to the extent that those activities relate to NDIS supports and services;

2. Engaging in activities that would be undertaken as a consultant, to the extent those activities relate to NDIS supports and services;

3. Engaging in activities that would be undertaken as an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

4. Engaging in activities in a non-service and non-support delivery role as a provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS;

5. Engaging in activities involving the administration and management of funding for supports under a Participants Plan;

6. Providing or be involved or assisting in the provision, to any NDIS Participant, of any support(s)or service(s) under a Participant’s Plan.

The delegate holds a reasonable belief that Mr Charchouh is not a suitable person to be involved in
supports and services to NDIS participants.

Where:
NDIS Participant means a participant within the meaning of section 9 of the NDIS Act
NDIS Provider means an NDIS Provider within the meaning of section 9 of the NDIS Act
Participant’s Plan means a plan within the meaning of section 9 of the NDIS Act

This banning order takes effect at 5.00pm on 2 November 2026 for a period of five (5) years and operates across all States and
Territories of Australia.

On 21 October 2026 a temporary banning order was made under section 73ZN(2)(b) of the National Disability Insurance Scheme Act 2013 prohibiting Erin Maree Cooper from providing, being involved, or assisting in the provision, to any NDIS participant, of any supports and/or services under a Participant’s Plan and which:-

a. Involves contact with NDIS participants, whether in a NDIS participant's home or elsewhere, and/or

b. Involves you accessing or receiving a NDIS participant's personal or financial information. 

In this instance:
• 'Contact' means face-to-face contact, oral communication, written communication or electronic communication, where they are physically touching the participant, speaking with, or building rapport with the participant
• NDIS Participant means a participant within the meaning of section 9 of the NDIS Act
• NDIS Provider means a NDIS Provider within the meaning of section 9 of the NDIS Act and
• Participant’s Plan means a plan within the meaning of section 9 of the NDIS Act. 

The banning order was made on the basis that Erin Maree Cooper has been convicted of an offence involving fraud or dishonesty. 

This banning order is temporary operating for (2) two years with effect at 5:00pm on 2 November 2026 and operates across all states and territories of Australia.

On 21 October 2026 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Wayne Stephen Savage from providing, being involved, or assisting in the provision, to any NDIS participant, of any supports and/or services under a Participant’s Plan and which:
 

a. Involves contact with NDIS participants, whether in an NDIS participant's home or elsewhere, and/or

b. Involves you accessing or receiving an NDIS participant's personal or financial information. 

In this instance:

• 'Contact' means face-to-face contact, oral communication, written communication or electronic communication, where you are physically touching the participant, speaking with, or building a rapport with the participant • NDIS Participant means a participant within the meaning of section 9 of the NDIS Act
• NDIS Provider means a NDIS Provider within the meaning of section 9 of the NDIS Act and
• Participant’s Plan means a plan within the meaning of section 9 of the NDIS Act.

 In making this banning order the delegate determined that Wayne Stephen Savage is not suitable to provide supports and services to NDIS participants as a result of his substantial criminal history and the risk this poses to people with disabilities. 

This temporary banning order takes effect from 5pm on 2 November 2026 for a period of ten (10) years and operates across all States and Territories of Australia.

On 21 October 2026 a permanent banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Zachary John Singleton from providing, being involved, or assisting in the provision, to any NDIS participant, of any supports and/or services under a Participants Plan and which:

a. Involves contact with NDIS participants, whether in an NDIS participant's home or elsewhere, and/or

b. Involves you accessing or receiving a NDIS participant's personal or financial information. In this instance:

• 'Contact' means face-to-face contact, oral communication, written communication or electronic communication, where you are physically touching the participant, speaking with, or building a rapport with the participant

• NDIS Participant means a participant within the meaning of section 9 of the NDIS Act

• NDIS Provider means a NDIS Provider within the meaning of section 9 of the NDIS Act and
• Participant’s Plan means a plan within the meaning of section 9 of the NDIS Act. 

In making this banning order the delegate formed a reasonable belief that Zachary John Singleton is not suitable to provide supports and services to people with disability, having regard to Zachary John Singleton’s criminal history. 

This banning order takes effect at 5:00pm on 2 November 2026 and operates across all states and territories of Australia.

Billy Naude has been banned pursuant to section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 as the delegate of the Commissioner formed a reasonable believe that Mr Naude is not suitable to provide, manage or be involved in supports or services for people with disability due to his conduct while providing support and services to a participant.

A permanent order prohibiting/restricting, Billy Naude and ABN 26 430 792 269, from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

a) The provision, to any NDIS participant, of any of the classes of support set out in rule 20(3) of NDIS (Provider Registration and Practice Standards) Rules 2018.
b) Undertaking any management role, as the key personnel of an NDIS provider as defined in section 11A of the NDIS Act.
c) Working in a non-service and support delivery role for an NDIS provider.
d) Engaging in activities that would be undertaken by a consultant for an NDIS provider, including services that involve assisting with, or advising on, applications under section 73C of the NDIS Act for registration as a registered NDIS provider.
e) Engaging in activities that would be undertaken by an approved quality auditor for an NDIS provider, including services that enable or facilitate the provision of supports or services for people with disability.
f) Being employed or otherwise engaged by an approved quality auditor, including as a member of the approved quality auditor's key personnel, to the extent that those activities relate to NDIS supports or services.

NDIS Participant means a participant within the meaning of section 9 of the NDIS Act;
NDIS Provider means a NDIS Provider within the meaning of section 9 of the NDIS Act; and
Participant’s Plan means a plan within the meaning of section 9 of the NDIS Act.

This banning order takes effect on 5:00pm AWST on 30 October 2026 for a permanent period and operates across all States and Territories of Australia.

On 12 October 2026, a banning order was made under section 73ZN(1)(b)(i) of the National Disability Insurance Scheme Act 2013 prohibiting Mr Robert Watt from engaging in NDIS-related activities for a period of five (5) years.

Mr Robert Watt is prohibited from:

1. Delivering supports or services to NDIS funded participants as outlined in section 20(3) of the Practice Standards Rules.
2. Engaging in activities that a member of key personnel of a NDIS provider, as defined in section 11A of the NDIS Act, would undertake to the extent that those activities relate to NDIS supports or services.
3. Engaging in activities that would be undertaken by a consultant for a NDIS provider, as outlined in section 73ZN(2C)(d) of the NDIS Act, to the extent that those activities relate to NDIS supports or services.
4. Engaging in activities that would be undertaken by an auditor, as defined in section 73U of the NDIS Act, for a NDIS provider to the extent that those activities relate to NDIS supports or services.
5. Engaging in activities in a non-service and non-support delivery role for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS.

NDIS Participant means a participant within the meaning of section 9 of the NDIS Act; and

NDIS Provider means a NDIS Provider within the meaning of section 9 of the NDIS Act; and

Participant’s Plan means a plan within the meaning of section 9 of the NDIS Act.

The Commissioner reasonably believes that Mr Watt contravened the NDIS Act by failing to maintain appropriate safeguards and supervision while providing supports to an NDIS participant. This resulted in care and support not being provided in a safe and competent manner to a participant who was under Mr Watt's care and supervision. The Commissioner formed the view that Mr Watt's conduct presents an ongoing risk to people with disability and that a banning order is necessary to protect NDIS participants.

This banning order takes effect on 30 October 2026 at 5:00 pm AEST and remains in force for five (5) years and operates across all States and Territories of Australia.

On 21 October 2026, a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Mr Dan Ian Kilian from, 

1. Engaging in the activities that a member of key personal of a National Disability Insurance Scheme (NDIS) provider would undertake, to the extent that those activities relate to NDIS supports or services.
2. Engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports and services.
3. Engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services.
4. Engaging in activities in a non-services and non-support delivery role for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports of services to people with disability in the NDIS.
5. Engaging in the provision or management of NDIS supports, services or funding to people with disability in the NDIS.
6. Providing or being involved or assisting in the provision, to any NDIS participant, of any support or supports under a participant’s plan.

7. Engaging in any work or providing or performing any service or services (whether paid or unpaid), for or on behalf of any NDIS provider, which may occasion any personal contact between you and a NDIS participant.

   Where:
   NDIS Participant means a participant within the meaning of section 9 of the NDIS Act
   NDIS Provider means an NDIS Provider within the meaning of section 9 of the NDIS Act
   Participant’s Plan means a plan within the meaning of section 9 of the NDIS Act

The banning order takes effect on 28 October 2026 at 5.00pm on a permanent basis and operates across all States and Territories of Australia.

On 21 October 2026, a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013, prohibiting Mr Emmanuel Kadzombe from providing supports and services to persons with disability in the National Disability Insurance Scheme (NDIS). 

Specifically, the order is permanent and prohibits Mr Emmanuel Kadzombe from being engaged in the following NDIS activities:

a) provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan.

b) provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan.

c) Engage in any work or provide or perform any service or services (whether paid or unpaid) for or on behalf of any NDIS Provider, which may occasion any personal contact between Mr Kadzombe and a NDIS Participant.

This permanent banning order takes effect at 5.00pm on 28 October 2026 and operates across all States and Territories of Australia.
