  • The search looks for individual, sole trader, trading names or company names.
  • Search for a business using a single word or partial word, as this may find spelling or name variations.
  • Search for a worker using first name or last name, not both.
  • If you've searched different possible spellings, action types or years and don't find any results, it's likely the provider or worker hasn't received any enforcement actions.

    Specifically, the 10-year order prohibits Mary Grace Gonzalo Dacumos from being engaged in the following NDIS activities: 

    a) Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan; 

    b) Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan; 

    This order takes effect from 5.00pm AEDT on 3 February 2026 and operates across all States and Territories of Australia.

    On 14 January 2026 a permanent banning order was made under section 73ZN(1)(b)(i) of the National Disability Insurance Scheme Act 2013 prohibiting Arlie Gibson from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS) for a period of permanently, including but not limited to: : 

    a. Delivering supports or services to NDIS funded participants; 

    b. Engaging in activities that a member of the key personnel of a NDIS provider would undertake, to the extent that those activities relate to NDIS supports or service; 

    c. Engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or service; 

    d. Engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services; and 

    e. Engaging in activities in a non-service and non-support delivery for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS. 

    This banning order takes effect at 5.00pm AEST on 29 January 2026 for a permanent period and operates across all States and Territories of Australia.

    On 15 January 2026, a banning order was made under subparagraph 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 prohibiting George Bakhos. The banning order prohibits George Bakhos from being engaged in the following:

     • providing or personally being involved in the provision, to any NDIS participant, of any support or services under a participant’s plan; or engaging in any work or providing or 

    • performing any supports or services (whether paid or unpaid), for or on behalf of any NDIS Provider.

    This banning order takes effect at 5.00pm AEDT on 26 January 2026 for a period of five years and operates across all States and Territories of Australia

    On 14 January 2026, a banning order was made to Mr Edward THOMAS, under section 73ZN(1)(a) of the National Disability Insurance Scheme Act 2013 permanently restricting/prohibiting All Care Provided, from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

    • engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

    • engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

    • engaging in activities in a non-service and non-support delivery role for an NDIS provider, the extent that those activities have a direct effect on the provision of NDIS funded supports or service to people with disability in the NDIS;

    • providing or being involved or assisting in the provision, to any NDIS participant, of any support or supports under a participant’s plan;

    • engaging in any work or providing or performing any service or services (whether paid or unpaid), for or on behalf of any NDIS provider, which may occasion any personal contact between you and an NDIS participant.

    On 15 January 2026, a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013. The banning order prohibits Kim Mills from: 

    1. Providing, being involved or assisting in the provision, to any NDIS Participant, of any support or services under a Participant’s Plan. 

    2. Engaging in any work or providing or performing any supports or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may provide access to a NDIS participant’s sensitive personal or financial information (such as banking details). 

    This permanent banning order takes effect 05:00pm AEST on 22 January 2026 and operates across all States and Territories of Australia.

    Achan Anyar

    On 23 December 2025, a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013, restricting Ms Achan Anyar from being engaged in the following NDIS activities: 

    1. The direct delivery, to any NDIS Participant, of any supports or services under a Participant’s Plan, otherwise than under the direct supervision of a person holding a clearance (within the meaning of the National Disability Insurance Scheme (Practice Standards—Worker Screening) Rules 2018) who has authority or responsibility for directing or controlling the activities of an NDIS provider. 

    Supervision means direct and in-person observations.

    NDIS Participant means a participant within the meaning of s 9 of the NDIS Act; 

    NDIS Provider means an NDIS Provider within the meaning of s 9 of the NDIS Act; 

    Participant’s Plan means a plan within the meaning of s 9 of the NDIS Act.

    The banning order comes into effect 05:00pm AEDT on 19 January 2026 for a period of 12 months and operates across all States and Territories of Australia.

    The revocation decision, which takes effect on 19 January 2026, was made on the basis that a delegate of the Commissioner was satisfied that Angel Disability Supports Pty Ltd is no longer suitable to be involved in the provision of supports or services for which Angel Disability Supports Pty Ltd is registered to provide. In forming this belief, the delegate had regard to Angel Disability Supports Pty Ltd’s failure to meet its registration obligations as a registered NDIS Provider.

    On 22 December 2025 a notice of revocation of registration was issued to Auspicare Pty Ltd under section 73P(1)(a) of theNational Disability Insurance Scheme Act 2013 (NDIS Act). The revocation takes effect from 19 January 2026. 

    Auspicare Pty Ltd can no longer operate as registered NDIS provider, including providing specialist behaviour support, specialist disability accommodation, plan management services, any class of support or service to a person whose NDIS plan is managed by the NDIA and implementing regulated restrictive practices. 

    The revocation decision was made on the basis that a delegate of the Commissioner reasonably believes that Auspicare Pty Ltd has contravened, is contravening, or is proposing to contravene the NDIS Act.  

    In forming this reasonable belief, the delegate had regard to outcomes of an audit undertaken of Auspicare Pty Ltd and which identified a number of major non-conformities with applicable NDIS Practice Standards and which Auspicare Pty Ltd is alleged to have failed to rectify.

    On 16 January 2024 a banning order was made under sections 73ZN(2)(a)(i) and 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013. On 15 January 2026 the original banning order was varied under section 73ZO of the National Disability Insurance Scheme Act 2013. The variation makes the original banning order permanent. 

    The order permanently prohibits Mumthaj Begam Kantara from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme, including but not limited to: 

    • engaging in activities that a member of the key personnel of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services; 

    • engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

    On 17 December 2025, a banning order was made under paragraph 73ZN (2)(a)(i) and (iii) of the National Disability Insurance Scheme Act 2013, prohibiting Shayne John Wallis from providing supports and services to persons with disability in the National Disability Insurance Scheme (NDIS). Specifically, the 10 year order prohibits Shayne John Wallis from being engaged in the following NDIS activities:

    a) Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;

    b) Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan;

    c) engaging in activities that a member of the key personnel (as defined under section 11A of the NDIS Act) of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;

    d) engaging in activities that would be undertaken as a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

    This banning order takes effect 5:00pm on 12 January 2026 for a period of 10 years and operates across all States and Territories of Australia.

    On 23 December 2025, a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013. The banning order prohibits and restricts Keshav Sharma from engaging in the following specified activities:

    a) Delivering supports or services to NDIS funded participants; 

    b) Engaging in activities that a member of key personnel of a NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services; 

    c) Engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services; 

    d) engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services; and/or 

    e) Engage in activities in a non-service and non-support delivery role for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS. 

    The banning order applies for a permanent period and operates across all states and territories of Australia. This order will take effect from 5:00PM AEST on 9 January 2026.

    This banning order takes effect on 9 January 2026 at 5.00pm ACDT for a period of three (3) years and operates across all States and Territories of Australia.

    c) engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

    On 11 December 2025 a permanent banning order against Mitchell David DUNSTAN was issued under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013. The banning order prohibits Mitchell David DUNSTAN from providing disability supports and services in the NDIS market and with people with disability.

    Specifically, the order prohibits Mitchell David DUNSTAN from engaging in the following activities: 

    a. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s plan; 

    b. Provide or be involved or assist in the provision, to any NDIS participant, of any service or services under a Participant’s Plan; and 

    c. Making, preparing, submitting or assisting with any claim, invoice or request for payment to the NDIA in relation to any supports or services provided under a Participant’s Plan. 

    NDIS Participant means a participant within the meaning of s 9 of the NDIS Act; 

    NDIS Provider means an NDIS Provider within the meaning of s 9 of the NDIS Act; 

    Participant’s Plan means a plan within the meaning of s 9 of the NDIS Act. 

    This permanent banning order takes effect on 5 January 2026 at 5.00pm AEDT and operates across all States and Territories of Australia
