To make an informed decision about the providers or workers you employ or work with, you can search their compliance record to see if they have received any current or past compliance or enforcement actions. More information about the actions included in the search.

  • The search looks for individual, sole trader, trading names or company names.
  • Search for a business using a single word or partial word, as this may find spelling or name variations.
  • Search for a worker using first name or last name, not both.
  • If you've searched different possible spellings, action types or years and don't find any results, it's likely the provider or worker hasn't received any enforcement actions.

    On 27 April 2026, a banning order was made under section 73ZN(1)(a) of the National Disability Insurance Scheme Act 2013 prohibiting Mon Cheri Gifts Pty Ltd T/A Diversity Models from engaging in the following NDIS activities: 

    1. Engaging in activities that a of a National Disability Insurance Scheme (NDIS) provider would undertake, to the extent that those activities relate to NDIS supports or services in the registered or unregistered market. 

    2. Engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services in the registered or unregistered market. 

    3. Engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services in the registered or unregistered market. 

    4. Engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports of services to people with disability in the NDIS in the registered or unregistered market. 

    5. Engaging in the provision or management of NDIS supports, services or funding to people with disability in the NDIS. 

    6. Providing or being involved or assisting in the provision, to any NDIS participant, of any support or supports under an NDIS participant’s plan. 

    7. Engaging in any work or providing or performing any service or services (whether paid or unpaid), for or on behalf of any NDIS provider, which may occasion any personal contact between Mon Cheri Gifts Pty Ltd and an NDIS participant. 

    Where: 

    NDIS Participant means a participant within the meaning of section 9 of the NDIS Act 

    NDIS Provider means an NDIS Provider within the meaning of section 9 of the NDIS Act 

    Participant’s Plan means a plan within the meaning of section 9 of the NDIS Act 

    This Order takes effect on 19 May 2026 at 5.00pm AEST and operates for a period of 2 years across all States and Territories of Australia

  • In force: Revocation of registration of registered NDIS provider - from 5pm 12 May 2026

    Mon Cheri Gifts Pty Ltd T/A Diversity Models

    On 28 April 2026, the revocation of registration order was made against Mon Cheri Gifts Pty Ltd T/A Diversity Models under section 73P(1)(e) of the National Disability Insurance Scheme Act 2013 prohibiting Mon Cheri Gifts Pty Ltd from being engaged in the following NDIS supports or services.

    Mon Cheri Gifts Pty Ltd can no longer operate as a registered NDIS provider and as such is prohibited from:

    • Undertaking all activities associated with registration as a registered NDIS provider and must not represent itself as a registered NDIS provider, as at the date the revocation takes effect.

    • Providing supports and services that require registration as set out in the National Disability Insurance Scheme (Provider Registration and Practice Standards) Rules 2018 including innovative community participation.

    • Providing supports and services to any NDIS participant whose plan is managed by the National Disability Insurance Agency.

    • Delivering activities that a registered NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services. 

    NDIS Participant means a participant within the meaning of section 9 of the NDIS Act; 

    NDIS Provider means an NDIS Provider within the meaning of section 9 of the NDIS Act; 

    Participant’s Plan means a plan within the meaning of section 9 of the NDIS Act. 

    This Order takes effect on 12 May 2026 at 5:00pm AEST and operates across all states and territories of Australia.

    The revocation decision was made on the basis that a delegate of the Commissioner is no longer satisfied that Ms Monique Jeremiah, key personnel of Mon Cheri Gifts Pty Ltd T/A Diversity Models, is suitable to be involved in the provision of supports or services for which she is registered to provide, having regard to any matters prescribed by the National Disability Insurance Scheme including the requirements under section 73V of the National Disability Insurance Scheme Act 2013.

    In forming this belief, the delegate considered Ms Jeremiah’s disregard for her obligations under the section 6(1)(b) (respect the privacy of people with a disability) and section 6(1)(d) (act with integrity, honesty and transparency) of the NDIS (Code of Conduct) Rules 2018

    On 27 April 2026, a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Ms Monique Jeremiah from being engaged in the following NDIS supports or services: 

    1. Engaging in activities that a member of the key personnel of a National Disability Insurance Scheme (NDIS) provider would undertake, to the extent that those activities relate to NDIS supports or services. 

    2. Engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services. 

    3. Engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services. 

    4. Engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports of services to people with disability in the NDIS. 

    5. Engaging in the provision or management of NDIS supports, services or funding to people with disability in the NDIS.
