A 24-hour report is the first short report you must submit within 24 hours of key personnel becoming aware of a serious incident (like an injury, abuse, or neglect)
Unauthorised restrictive practice
An unauthorised restrictive practice is a specific types of incident involving the use of a restrictive practice without proper authorisation.
5-day report
A 5-day report is a more detailed reports with extra assessment information. You must submit this within 5 business days of the key personnel being advised about the incident.
Impacted person
The Impacted person is the NDIS participant who was harmed or impacted by the incident
Associated person
An associated person is the person who may have caused the incident such as a staff member associated with the incident. This is also known as 'subject of allegation'.
Safeguarding or immediate actions
Safeguarding or immediate actions are the steps your staff took right away to keep the participant and others safe, such as applying first aid, calling an ambulance, or suspending a staff member.
Report record
A report record contains the details captured about an incident from a Provider. You can click on a report record to view its status or perform actions associated to the report such as completing a 5 day report or adding more information
Request for information
A request for information is an official request sent through the portal when the Commission needs you to provide more details or attach specific documents (like medication charts or hospital discharge summaries) to your report.

An unauthorised restrictive practice report record contains information about a restrictive practice used on a participant.
