About this guide
This guide is for providers who are reporting an incident with a participant caused by your own staff or another registered provider.
Reportable Incidents are serious events or incidents that you are legally required to tell us about.
You must report incidents about:
- death of a participant
- a serious injury
- abuse
- unlawful sexual or physical contact
- sexual misconduct
- neglect
- an unauthorised restrictive practice.
Notify us on time
You must tell us about an incident on time.
If you are late, we might give you an infringement notice.
An infringement notice is a fine you must pay.
We might also take other compliance actions.
Compliance actions are steps we take to make you follow the rules.
