Application form

Why apply?

The NDIS Quality and Safeguards Commission is responsible for registering providers to deliver NDIS supports and services in New South Wales and South Australia.

Your organisation's registration requirements are determined by:

  1. the size of your organisation
  2. the level of complexity in the supports or services that you provide or intend to provide to participants.

This ensures that the requirements are tailored to the risk and complexity of the supports and services you provide.

What you will need whilst working through this application

3. Self-assessment against the required standards

Based on the answers you provide, you will be asked to explain how your organisation meets the requirements identified. This includes the opportunity to upload supporting evidence.

4. Suitability

You are required to disclose any past bankruptcies, convictions or other matters which may affect your suitability to deliver supports and services to participants.
