   • Reduction in the scope of services provided.
   • Exiting specific markets or service areas.
   • Ceasing operations entirely.
      
2. A breakdown of this data by:
   • State and Territory where the providers are registered.
   • The type of services reduced or ceased (e.g., Supported Independent Living, therapy services, plan management).
   • The number of participants affected
   • If the provider identified any reason for the change
      
3. The timeframe for this data: the last two financial years (or the most recent available period).
    
4. Any policy or procedure documents used by the NDIS Commission to manage or respond to notifications from providers about changes in their ability to operate, including guidelines for supporting participants affected by market exits.

Register of NDIS officers approved as monitoring (inspectors) and investigators who have been issued with an identification under 35(1) of the Regulatory Powers Act 2014. Number of cards not returned under 35(3) of the Act. Names should be removed as only number of those issued and number of staff in each group are required for the following staff groups:
