Submitting behaviour support plans and reports

How to lodge behaviour support plans

Any behaviour support plans containing regulated restrictive practices must be lodged by specialist behaviour support providers with the NDIS Commission.

Lodgement occurs in the NDIS Commission portal and requires a PRODA account.

Lodgement involves entering details about the participant, the regulated restrictive practices and any implementing providers and attaching copies of the behaviour support plan and where relevant the functional behavioural assessment.

For more information see the Lodging a behaviour support plan user guide.

The format of the behaviour support plan

NDIS behaviour support practitioners can use the NDIS Commission’s behaviour support plan templates, their own template, or any template specifically required by the state or territory restrictive practice authorisation body.

The NDIS Commission has undertaken a review of the Interim and Comprehensive Behaviour Support Plan Templates. Practitioners and providers are encouraged to use the revised resources:

As a transitional arrangement, copies of the previous templates (V2.0) remain available for use together with a summary of findings which informed the new templates (V3.0).

This transitional arrangement will end on 30 June 2024.

The following documents will no longer be available on the website effective 01 July 2024:

  • The Interim Behaviour Support Template V2.0
  • The Comprehensive Behaviour Support Plan V2.0
  • BSP Template Review: Summary of Findings
