On 2 June 2025, a banning order was made under 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 restricting Muhammad Ali from being involved in providing NDIS supports or services to NDIS funded participants under the age of 18 years.

The banning order takes effect on 23 June 2025 at 5:00PM AEST for a period of four (4) years across all States and Territories of Australia.

On 2 June 2025 a banning order was made under 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Rosemary Jane Hughes from:

• Being engaged, or otherwise employed, in any role where they are responsible for the provision of any NDIS supports or services to NDIS participants; and

• Having unsupervised contact with NDIS participants as part of the provision of NDIS supports or services. 

   

In this instance: 

• Contact includes physical contact, face-to-face contact, oral communication, written communication and electronic communication; and
• Supervision means direct and in-person observation. 

The banning order takes effect on 23 June 2025 at 5:00PM ACST for a period of four (4) years across all States and Territories of Australia.

On 27 May 2025, a permanent banning order was issued to Horizon Solsolutions Australia Pty Ltd under 73ZN (1)(b)(i-iii) of the National Disability Insurance Scheme Act 2013. Specifically, Horizon Solsolutions Australia is restricting/prohibiting from being engaged in the following NDIS activities:

1. engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;
2. engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services;
3. engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS;
4. engaging in the provision or management of NDIS supports, services or funding to people with disability in the NDIS.

The banning order takes effect on 7 June 2025 at 9.00am AEST and operates across all States and Territories of Australia.

On 27 May 2025, a permanent banning order was issued under 73ZN(2)(b) of the National Disability Insurance Scheme Act 2013 prohibiting Muhammad Latif from being engaged in the following NDIS activities:

1. engaging in activities that a member of the key personnel of an National Disability Insurance Scheme (NDIS) provider would undertake, to the extent that those activities relate to NDIS supports or services;
2. engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;
3. engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services;
4. engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS;
5. engaging in the provision or management of NDIS supports, services or funding to people with disability in the NDIS.

The banning order takes effect on 7 June 2025 at 9.00am AEST and operates across all States and Territories of Australia.

On 23 May 2025, a banning order was made under sections 73ZN(1)(a), 73ZN(1)(b)(i)-(iii) of the National Disability Insurance Scheme Act 2013 permanently restricting/prohibiting Nandish Services Pty Ltd t/a Safe Hands / Beeu Support Coordination, from engaging directly or indirectly, in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. engaging in activities that a member of the key personnel of a National Disability Insurance Scheme (NDIS) provider would undertake, to the extent that those activities relate to NDIS supports or services; 

2. engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services; 

3. engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services; 

4. engaging in activities in a non-service and non-support delivery role for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS; 

5. engaging in the provision or management of NDIS supports, services or funding to people with disability in the NDIS. 

The banning order is effective from 5:00pm AEST on 6 June 2025 and operates across all States and Territories of Australia.

On 28 May 2025, a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013, restricting Renee Disisto from the following activities:

• providing or personally being involved in the provision, to any NDIS participant, of any support or supports under a participant’s plan; or
• engaging in any work or providing or performing any supports or services (whether paid or unpaid), for or on behalf of any NDIS Provider.

The banning order is effective on 6 June 2025 at 9:00am ACST for a period of two (2) years and operates across all States and Territories of Australia.

On 23 May 2025 a banning order of Jaideep Singh was made, under s73ZN(2)(a)(i) – (iii) of the National Disability Insurance Scheme 2013, restricting/prohibiting Jaideep Singh from engaging, directly or indirectly, in the provision or management of NDIS supports, services or funding to people with a disability in the NDIS, including but not limited to:

1. engaging in activities that a member of the key personnel of a National Disability Insurance Scheme (NDIS) provider would undertake, to the extent that those activities relate to NDIS supports or services; 

2. engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services; 

3. engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services; 

4. engaging in activities in a non-service and non-support delivery role for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS; 

5. engaging in the provision or management of NDIS supports , services or funding to people with disability in the NDIS. 

The banning order is effective on 6 June 2025 at 5:00pm AEST for a period of three (3) years and operates across all States and Territories of Australia. 

On 27 May 2025, a banning order was made under sections 73ZN(2)(a)(i) and 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 permanently restricting/prohibiting Rabih Chamma from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to undertaking the following activities:

• engaging in activities that a member of the key personnel of a NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;
• engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services;
• engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services;
• engaging in activities in a non-service and non-support delivery role for a NDIS provider, the extent that those activities have a direct effect on the provision of NDIS funded supports or service to people with disability in the NDIS.
• providing or being involved or assisting in the provision, to any NDIS participant, of any support or supports under a participant’s plan;
• engaging in any work or providing or performing any service or services (whether paid or unpaid), for or on behalf of any NDIS provider, which may occasion any personal contact between you and a NDIS participant.

The banning order takes effect on 3 June 2025 at 5.00pm AEST and operates across all States and Territories of Australia.

On 27 May 2025, a banning order was made under section 73ZN(1)(a) of the National Disability Insurance Scheme Act 2013 permanently prohibiting Reaching Out Services Pty Ltd from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to undertaking the following activities: 

• engaging in any activities that would be delivering support and services to NDIS funded participants;
• engaging in activities that a NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;
• engaging in activities that would be undertaken as a consultant, to the extent that those activities relate to NDIS supports or services;
• engaging in activities that would be undertaken as an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services; and
• engaging in activities in a non-service and non-support delivery role as a provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS. 

The banning order takes effect on 3 June 2025 at 5.00pm AEST and operates across all States and Territories of Australia.

On 26 May 2025 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 restricting/prohibiting Taimi Keegan from being involved in the handling or managing of any financial matters or personal finances for participants of the scheme. 

The banning order is effective from 5:00pm AEST on 30 May 2025 for a period of one (1) year and operates across all States and Territories of Australia.

On 13 May 2025 a banning order was made under Section 73ZN(2)(a)(i) and 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013, permanently restricting/prohibiting Michelle Leanne Lindsay from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

• engaging in activities that a member of the key personnel of a National Disability Insurance Scheme (NDIS) provider would undertake, to the extent that those activities relate to NDIS supports or services;

• engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services;

• engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services;

• engaging in activities in a non-service and non-support delivery role for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS;

• engaging in the provision or management of NDIS supports, services or funding to people with disability in the NDIS.

The banning order is effective from 5:00pm AEST on 27 May 2025 and operates across all States and Territories of Australia.

On 12 May 2025, a permanent banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013. The banning order permanently prohibits and restricts Riley Geppert from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. Delivering supports or services to NDIS funded participants; 

2. Engaging in activities that a member of the key personnel of a NDIS provider would undertake, to the extent that those activities relate to NDIS supports or service; 

3. Engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or service; 

4. Engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services; 

5. Engaging in activities in a non-service and non-support delivery role for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS. 

The banning order will be effective from 5pm AEST on 26 May 2025, across all States and Territories of Australia.

n 12 May 2025, a permanent banning order was made under section 73ZN(1)(b)(i) of the National Disability Insurance Scheme Act 2013. 

The banning order permanently prohibits and restricts Independent Lifestyles Riverina Pty Ltd from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. Engaging in activities of, and as, a NDIS provider; 

2. Engaging in activities that a member of the key personnel of a NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services; 

3. Engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services; 

4. Engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services; and/or 

5. Engaging in activities in a non-service and non-support delivery role for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS. 

The banning order will be effective from 5pm AEST on 26 May 2025, across all States and Territories of Australia.

On 24 April 2025 a permanent banning order was made under section 73ZN(1)(b)(i) of the National Disability Insurance Scheme Act 2013. The banning order prohibits and restricts Duggal Services Pty Ltd from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. engaging in activities that would be undertaken by a NDIS provider including delivering supports and services, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS.
2. engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services; and
3. engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services.

The permanent banning order is effective from 5:00pm AEST on 21 May 2025, across all States and Territories of Australia.

On 12 May 2025 the delegate signed a banning order of limited application under Section 73ZN(1)(b)(i) National Disability Insurance Scheme Act 2013 (Cth) restricting Tough Love 45 Pty Ltd from being engaged in the following NDIS activities:

1. Delivering supports or services to NDIS funded participants;

2. Engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services;

3. Engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services; and/or

4. Engaging in activities in a non-service and non-support delivery role for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS. 

The banning order is effective from 5pm AEST on 19 May 2025 for a period of 10 (10) years, across all States and Territories of Australia.

On 12 May 2025 the delegate signed a banning order of limited application under Section 73ZN(2)(a)(i) National Disability Insurance Scheme Act 2013 (Cth) restricting Karen Tough from being engaged in the following NDIS activities: 

1.Delivering supports or services to NDIS funded participants; 

2. Engaging in activities that a member of the key personnel of a NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services; 

3.Engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services; 

4.Engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services; and/or 

5.Engaging in activities in a non-service and non-support delivery role for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS. 

The banning order is effective from 5pm AEST on 19 May 2025 for a period of 10 (10) years, across all States and Territories of Australia.

On 2 May 2025 a banning order was made under Section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 prohibiting and restricting Daina Alopa from engaging in the unsupervised provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. Delivering supports or services to NDIS funded participants unless physical present supervision;
2. engaging in activities that a member of the key personnel of a NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;
3. engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services;
4. engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services; and
5. engaging in activities in a non-service and non-support delivery role for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS. 

The banning order is effective from 5:00pm AEST on 19 May 2025 and operates across all States and Territories of Australia.

On 12 May 2025 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Khushpreet Singh from being engaged in the following activities: 

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;
2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan.

The banning order is effective from 5:00pm ACST 19 May 2025 for a period of five (5) years and operates across all States and Territories of Australia.

On 23 April 2025 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Gurcharan Bhathal from: 

1.Provide or be involved or assist in the provision, to any NDIS participant, of any support(s) or service(s) under a Participant’s Plan.

2.Engage in any work or provide or perform any support(s) or service(s) (whether paid or unpaid), for or on behalf of any NDIS Provider. 

The banning order is effective for a period of one (1) year from 05:00pm ACST from 16 May 2025, across all States and Territories of Australia.

On 8 May 2025, a banning order was varied under section 73ZO of the National Disability Insurance Scheme Act 2013, to extend the current banning order expiry for a further six (6) months and will take effect from 16 May 2025.

The existing banning order for Mana Support is to be extended for a further six (6) months, during which time Mana Support are prohibited from providing supports or services to participants funded by the National Disability Insurance Scheme (NDIS), specifically:

1. where there is, or is likely to be, an interim or ongoing need to use a regulated restrictive
   practice (RRP) in relation to the participant; or
2. where the participant has a behaviour support plan (BSP) that is lodged with the NDIS Commission.

This variation to the banning order will take effect from 5:00pm AEST on 16 May 2025 for a period of six months, ending on 16 November 2025 across all States and Territories of Australia.

On 30 April 2025, a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013. The banning order restricts Briarsha Lewis from being engaged in the following NDIS activities:

1. providing unsupervised NDIS supports or services to people with disability in the NDIS and

2. to undertake supervised work at all times, and/or work in a role that does not require supporting people with a disability unsupervised. 

The banning order is for a period of one (1) year and will be effective from 5pm AEST on 16 May 2025 across all States and Territories of Australia.

On 13 May 2025, a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 permanently restricting/prohibiting Simon William Barrett from the following activities related to the provision of NDIS funded supports and services to NDIS participants:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or under a Participant’s Plan
2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan
3. Engage in any or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and a NDIS Participant.

This banning order is effective from 5:00pm ACST on 16 May 2025 across all States and Territories of Australia.

On 13 May 2025 a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 restricting/prohibiting Michael Lynch from being engaged in the provision of supports or services to any NDIS participants in the following activities:

1. Provide or be involved or assist in the provision of any support or supports under a NDIS participant’s plan;
2. Provide or be involved or assist in the provision of any service or services under a NDIS participant’s plan;
3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS provider, which may occasion any personal contact with any NDIS participant.

The banning order is effective from 5:00pm AEST 16 May 2025 for a period of two (2) years and operates across all States and Territories of Australia.

On 28 April 2025, a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Mr Alzavian Chicken from:

1. Providing or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan; and
2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan. 

The banning order is effective for a period of five (5) years from 5:00pm AEST on 12 May 2025, across all States and Territories of Australia.

On 28 April 2025, a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Luke David Waqa from engaging in the provision of NDIS supports or services to people with disability in the National Disability Insurance Scheme, including but not limited to:

1. Providing or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan; and
2. Providing or be involved or assist in the provision, to any NDIS participant, of any service or services under a Participant’s Plan.

The banning order is for a period of five (5) years, effective from 5:00pm AEST on 12 May 2025, across all States and Territories of Australia.

On 28 April 2025, a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 prohibiting Brodie Baxter from engaging in the provision of NDIS supports or services to people with disability in the National Disability Insurance Scheme, including but not limited to:

1. Providing or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan; and
2. Providing or be involved or assist in the provision, to any NDIS participant, of any service or services under a Participant’s Plan.

The banning order is for a period of 1 year, effective from 5:00pm AEST on 12 May 2025, across all States and Territories of Australia.

On 2 May 2025 a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013, prohibiting Zainab Dauda from being engaged in the following NDIS activities:

• providing or personally being involved in the provision, to any NDIS participant, of any support or supports under a participant’s plan; or

• engaging in any work or providing or performing any supports or services (whether paid or unpaid), for or on behalf of any NDIS Provider.

The banning order is effective from 5:00pm AEST 9 May 2025 for a period of twelve (12) months and operates across all States and Territories of Australia.

On 24 April 2025, a banning order was made under section 73ZN(2)(a) of the National Disability and Insurance Scheme Act 2013. The banning order prohibits Sarah Abu Lebdeh from engaging in the following NDIS activities:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan;

3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and an NDIS Participant. 

The Banning Order is for a period of two (2) years, effective from 5pm AEST on 9 May 2025, across all States and Territories of Australia.

On 2 May 2025, a banning order was made under section 73ZN(2)(a) of the National Disability and Insurance Scheme Act 2013. The banning order prohibits Ahmad Rashad Nadir from engaging in the following NDIS activities:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan;

3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and an NDIS Participant. 

The Banning Order is for a period of two (2) years, effective from 5pm AEST on 9 May 2025, across all States and Territories of Australia.

On 2 May 2025 a permanent banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting/restricting Mr Joseph Raymond Saba from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1) Delivering supports or services to NDIS funded participants;

2) Engaging in activities that a member of the key personnel of a NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;

3) Engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services;

4) Engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services; and/or

5) Engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS. 

The banning order is in place for a permanent period effective from 5:00PM AEDT on 9 May 2025, across all States and Territories of Australia.

On 15 April 2025 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013. The banning order prohibits Robert Harold Keene from being involved in activities in any role that provides NDIS supports or services that involves direct or indirect contact with NDIS participants, which means:

1. physically touching a person with disability; or
2. having contact with multiple NDIS funded participants as part of the direct delivery of a specialist disability support or service; or
3. building a rapport with NDIS funded participants as an ordinary part of the performance of normal duties whether face-to-face, or via oral communication, written communication, or electronic communication. 

The banning order is for a period of four (4) years effective from 5:00pm AWST on 30 April 2025, across all States and Territories of Australia.

On 15 April 2025 a banning order was made under 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 . 

The banning order prohibits Katreece Milli Knaap from being involved in providing NDIS supports or services to NDIS funded participants, which in this instance means: 

1. being engaged, or otherwise employed, in any role (paid or unpaid) where you are solely responsible for the provision of NDIS supports or services to NDIS participants; or
2. having contact with NDIS participants as part of the provision of NDIS supports or services without supervision. 

In this instance, supervision means direct and in-person observation.  

The banning order is effective for a period of four (4) years from 5:00pm AEST on 30 April 2025, across all States and Territories of Australia.

On 15 April 2025 a banning order was made under 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013. The banning order prohibits Jade Sarah VANDENBERG from being involved in the provisions of supports or services to people with a disability in the NDIS. Specifically, the Delegate made the order restricting Ms Vandenberg for a period of five (5) years from: 

1. Being engaged, or otherwise employed, in any role where you are solely responsible for the provision of any NDIS supports or services to NDIS participants; and
2. Having contact with NDIS participants without supervision, as part of the provision of NDIS supports or services. 

In this instance: 

• Contact includes physical contact, face-to-face contact, oral communication, written communication and electronic communication; and
• Supervision means direct and in-person observation. 

The banning order is for a period of five (5) years, effective from 5:00pm ACST 30 April 2025, across all States and Territories of Australia.

On 15 April 2025 a banning order was made under section 73ZN(2)(a)(iii) of the NDIS Act 2013. 

Ephrem Berihu MEHARI is not suitable to be involved in directly providing NDIS supports or services (paid or unpaid) to NDIS Participants. The banning order prohibits Ephrem Mehari from: 

1. physically touching a person with disability; or
2. having contact with multiple NDIS funded participants as part of the direct delivery of a specialist disability support or service; or
3. building a rapport with NDIS funded participants as an ordinary part of the performance of normal duties whether face-to-face, or via oral communication, written communication, or electronic communication. 

The banning order is for a period of three (3) years, effective at 5:00pm AEST on 30 April 2025, across all States and Territories of Australia.

On 15 April 2025 a banning order was made under 73ZN(2)(a)(iii) of the NDIS Act 2013. Serina Anne Harnack is not suitable to be involved in the provisions of supports or services to people with a disability in the NDIS. The banning order restricts Serina Anne Harnack from:

1. Being engaged, or otherwise employed, in any role where you are solely responsible for the provision of any NDIS supports or services to NDIS participants; and
2. Having contact with NDIS participants without supervision, as part of the provision of NDIS supports or services. 

In this instance: 

• Contact includes physical contact, face-to-face contact, oral communication, written communication and electronic communication; and
• Supervision means direct and in-person observation. 

The banning order is for a period of three (3) years, effective at 5:00pm ACST on 30 April 2025, across all States and Territories of Australia.

On 15 April 2025 a permanent banning order was made under 73ZN(2)(a)(iii) of the NDIS Act 2013 of the National Disability Insurance Scheme Act 2013. The banning order permanently prohibits Timothy James Grady from:

1. being involved in the provision of NDIS supports or services to NDIS participants under 16 years old.

The banning order takes effect on 30 April 2025 at 5:00pm AEST permanently and operates across all States and Territories of Australia.

On 15 April 2025 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013. The banning order prohibits Lisa Anne Norris from:

1. Being engaged, or otherwise employed, in any role where you are solely responsible for the provision of any NDIS supports or services to NDIS participants; and
2. Having contact with NDIS participants without supervision, as part of the provision of NDIS supports or services. 

In this instance: 

• Contact includes physical contact, face-to-face contact, oral communication, written communication and electronic communication; and
• Supervision means direct and in-person observation. 

The banning order takes effect on 30 April 2025 at 5:00pm ACST for a period of three (3) years and operates across all States and Territories of Australia.

On 2 April 2025 a banning order was made 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013.

The banning order prohibits Zoe Amy Maree Hurrell from being involved in directly providing NDIS supports or services to NDIS funded participants, which in this instance means: 

1. physically touching a person with disability; or
2. having contact with multiple NDIS funded participants as part of the direct delivery of a specialist disability support or service; or
3. building a rapport with NDIS funded participants as an ordinary part of the performance of normal duties whether face-to-face, or via oral communication, written communication, or electronic communication. 

The banning order takes effect on 24 April 2025 at 5:00pm ACST for a period of three (3) years and operates across all states and territories of Australia. 

On 7 April 2025 a permanent banning order was made under 73ZN(2)(a)(iii) of the NDIS Act of the National Disability Insurance Scheme Act 2013. The banning order permanently prohibits Carla Judith Smith from: 

1. Being engaged, or otherwise employed, in any role where you are solely responsible for the provision of NDIS supports or services to NDIS participants; and
2. Having unsupervised contact with NDIS participants as part of the provision of NDIS supports or services. 

In this instance: 

3. Contact includes physical contact, face to face contact, oral communication, written communication and electronic communication; and
4. Supervision means direct and in-person observation. 

The banning order is effective from 5:00pm ACST on 24 April 2025, across all States and Territories of Australia.

On 15 April 2025 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013. The banning order prohibits Feterika Faleono, also known as Feterika Terry Faleono for a period of 2 years from engaging in any activities relating to the provision of delivering supports and services to people with disability, including NDIS funded participants and in particular, the following:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support(s) or service(s) under a Participant’s Plan

2. Engage in any work or provide or perform any support(s) or service(s) (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and a NDIS Participant.

The banning order is for a period of two (2) years effective from 5:00pm AEST on 24 April 2025, across all States and Territories of Australia.

On 7 April 2025 a banning order was made under 73ZN(2)(a)(iii)  of the NDIS Act] of the National Disability Insurance Scheme Act 2013 . The banning order prohibits Melissa Robyn Smits from: 

1. Being engaged, or otherwise employed, in any role where you are solely responsible for the provision of any NDIS supports or services to NDIS participants; and
2. Having contact with NDIS participants without supervision, as part of the provision of NDIS supports or services. 

In this instance: 

3. Contact includes physical contact, face-to-face contact, oral communication, written communication and electronic communication; and
4. Supervision means direct and in-person observation. 

The banning order takes effect on 24 April 2025 at 5:00pm ACST for a period of five (5) years and operates across all States and Territories of Australia.

On 14 April 2025 a permanent banning order was made under section 73ZN(1)(a)(iii) of the National Disability Insurance Scheme Act 2013. The banning order permanently prohibits Thomas Fanous from:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;
2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan;
3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and a NDIS Participant. 

The banning order is effective on 24 April 2025 at 5.00pm AEST across all States and Territories of Australia.

On 15 April 2025, a banning order was made pursuant to section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 prohibiting Lucien Bwihambi from engaging in the following activities:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan; and
2. Provide or be involved or assist in the provision, to any NDIS participant, of any service or services under a Participant’s Plan.

The banning order takes effect on 24 April 2025 at 5.00pm AEST for a period of 18 months, across all States and Territories of Australia.

On 25 February 2025 a permanent banning order was made under 73ZN(2)(a)(iii) of the NDIS Act of the National Disability Insurance Scheme Act 2013. The banning order prohibits Luke Tham Shengen from being involved in the provision of supports or services to people with disability. 

Specifically, the Delegate made the order permanently prohibiting Luke Tham Shengen from providing NDIS supports or services in circumstances involving direct and indirect contact with NDIS participants, which in this instance means: 

1. physically touching a person with disability; or
2. having contact with multiple NDIS funded participants as part of the direct delivery of a specialist disability support or service; or
3. building a rapport with NDIS funded participants as an ordinary part of the performance of normal duties whether face-to-face, or via oral communication, written communication, or electronic communication. 

The banning order is effective from 5.00pm AWST on 24 April 2025, across all States and Territories of Australia.

On 23 April 2025 a banning order was made under 73ZN(2)(a)(iii) of the NDIS Act of the National Disability Insurance Scheme Act 2013. The banning order prohibits Chantelle Veronica Roberts from being involved in the provisions of supports or services to people with a disability in the NDIS. The banning order restricts Chantell Veronica Roberts from: 

• Being engaged, or otherwise employed, in any role where you are solely responsible for the provision of any NDIS supports or services to NDIS participants; and
• Having contact with NDIS participants without supervision, as part of the provision of NDIS supports or services. 

In this instance: 

• Contact includes physical contact, face-to-face contact, oral communication, written communication and electronic communication; and
• Supervision means direct and in-person observation. 

The banning order takes effect from 5pm AWST on 23 April 2025 for a period of four (4) years, across all States and Territories of Australia.  

On 16 April 2025 a permanent banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013. The banning order permanently prohibits Amara Jaroudi from being engaged in the following activities:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;
2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan;
3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and a NDIS Participant.

The banning order takes effect permanently on 22 April 2025 at 5.00pm AEST and operates across all States and Territories of Australia. 

On 2 April 2025, a banning order was made under section 73ZN(2)(a)(iii) of the NDIS Act of the National Disability Insurance Scheme Act 2013. The banning order prohibits Sally Denise Lincoln from being involved in the provision of supports or services to people with disability. Specifically, prohibiting Sally Lincoln from the following activities:

1. Access to any NDIS participant’s sensitive personal or financial information (such as banking details); and
2. Provision of any activities related to the management or use of NDIS plan funds, including handling of financial information relating to NDIS plans.

The banning order is for a period of four (4) years, effective from 5pm ACST on 21 April 2025, across all States and Territories of Australia.

On 7 April 2025, a banning order was made under sections 73ZN(2) (i) of the National Disability Insurance Scheme Act 2013. 

The banning order prohibits Joshua Anthony Mijas from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme, including but not limited to: 

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan; and
2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan. 

The banning order is for a period of two (2) years, effective from 5:00pm AEST on 21 April 2025, across all States and Territories of Australia.

On 2 April 2025 a banning order was made under 73ZN(2)(a)(iii) of the NDIS Act of the National Disability Insurance Scheme Act 2013. The banning order prohibits Billie-Jo Neale-Hamilton from being involved in the provision of NDIS supports or services that involves direct contact to people with disability. 

Specifically, the Delegate made the order prohibiting Billie-Jo Neale-Hamilton from the following activities: 

1. physically touching a person with disability; or
2. having contact with multiple NDIS funded participants as part of the direct delivery of a specialist disability support or service; or
3. building a rapport with NDIS funded participants as an ordinary part of the performance of normal duties whether face-to-face, or via oral communication, written communication, or electronic communication. 

The banning order is effective for a period of five (5) years from 5:00pm ACST on 21 April 2025, across all States and Territories of Australia.

On 17 April 2025 a banning order was made under 73ZN(2)(a)(iii) of the NDIS Act of the National Disability Insurance Scheme Act 2013. The banning order prohibits Tanikah-Jemma Helen Thompson from providing supports or services to people with a disability in the NDIS.  The banning order restricts Ms Thompson for a period of four (4) years from: 

• Being engaged, or otherwise employed, in any role where you are solely responsible for the provision of any NDIS supports or services to NDIS participants; and
• Having contact with NDIS participants without supervision, as part of the provision of NDIS supports or services. 

In this instance: 

• Contact includes physical contact, face-to-face contact, oral communication, written communication and electronic communication; and
• Supervision means direct and in-person observation. 

The banning order is effective from 5pm ACST on 17 April 2025 for a period of four (4) years and operates across all States and Territories of Australia.

On 17 April 2025 a banning order was made under 73ZN(1)(b)(iii) of the NDIS Act of the National Disability Insurance Scheme Act 2013.

The banning order prohibits Enande Mzaliwa from being involved in the provision of supports or services to people with a disability in the NDIS, specifically the order was made restricting Enande Mzaliwa from providing NDIS supports or services involving direct contact with NDIS participants.   

Direct contact means physical contact, face-to-face contact, oral communication, written communication and electronic communication.

The banning order takes effect from 5pm ACST on 17 April 2025 for a period of four (4) years, across all States and Territories of Australia.

On 17 April 2025 a banning order was made under 73ZN(2)(a)(iii) of the NDIS Act of the National Disability Insurance Scheme Act 2013. The banning order prohibits Cristy Lee-Anne Yovich from being involved in the provisions of supports or services to people with a disability in the NDIS.  Specifically, the order restricts Cristy Lee-Anne Yovich from:

• Being engaged, or otherwise employed, in any role where you are solely responsible for the provision of any NDIS supports or services to NDIS participants; and
• Having contact with NDIS participants without supervision, as part of the provision of NDIS supports or services. 

In this instance: 

• Contact includes physical contact, face-to-face contact, oral communication, written communication and electronic communication; and
• Supervision means direct and in-person observation. 

The banning order takes effect from 5pm AWST on 17 April 2025 for a period of five (5) years, across all States and Territories of Australia.

On 18 March 2025 a banning order for Donna Lee MILROY was made, under Section 73ZN(2)(a)(i), 73ZN(2)(a)(iii) and section 73ZN (2)(b) of the National Disability Insurance Scheme Act 2013. 

The banning order permanently restricts/prohibits Donna Lee MILROY from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. engaging in activities that a member of the key personnel of a NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services; 

2.  engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services; 

3.engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services; 

4.engaging in activities in a non-service and non-support delivery role for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS; 

5.engaging in the provision or management of NDIS supports, services or funding to people with disability in the NDIS. 

The banning order is effective from 5:00pm (AEST) on 15 April 2025 and operates across all States and Territories of Australia.

On 1 April 2025 a banning order was made under 73ZN(2)(a)(iii) of the NDIS Act of the National Disability Insurance Scheme Act 2013 prohibiting Donna Louise Tebble from: 

1.being involved in any NDIS supports or services to NDIS funded participants. 

The banning order is for a period of four (4) years, effective from 5:00pm ACST on 14 April 2025, across all States and Territories of Australia.

On 3 April 2025 a permanent banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013. The banning order prohibits Victor Hugo Gatica Guajardo from: 

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan.  

The banning order is effective from 5.00pm AEST on 11 April 2025, across all States and Territories of Australia.

On 27 March 2025 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013. The banning order restricts/prohibits Theo Papageorgakis from being engaged in activities related to the provision of NDIS funded supports and services to NDIS participants. 

Theo Papageorgakis is prohibited from:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;
2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan; and
3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and a NDIS Participant.

The banning order is effective for a period of two (2) years from 5:00pm ACST on 10 April 2025, across all States and Territories of Australia.

On 18 March 2025, a two-year banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013. The banning order prohibits Raju Lamichhane from:

1. providing or personally being involved in the provision, to any NDIS participant, of any support or supports under a participant’s plan; or

2.engaging in any work or providing or performing any supports or services (whether paid or unpaid), for or on behalf of any NDIS Provider.

The banning order is effective from 09:00am (AEDT) on 4 April 2025 and operates across all States and Territories of Australia.

On 4 April 2025 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013. The banning order restricts/prohibits Maria Crestani from being engaged in the following activities related to the provision of NDIS funded supports and services to NDIS participants for a period of five (5) years:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or under a Participant’s Plan
2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan
3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and a NDIS Participant.

The banning order is for a period of five (5) years effective from 5:00pm AEST on 4 April 2025, across all States and Territories of Australia.

On 3 April 2025 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013. The banning order prohibits Shane Darren Carle from:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan.

The banning order is for a period of five (5) years, effective from 5.00pm AWST on 4 April 2025, across all States and Territories of Australia.

On 3 April 2025, a banning order originally issued on 16 December 2024 was varied. The banning order was varied under section 100 of the National Disability Insurance Scheme Act 2013 prohibiting Mr Nicholas (Nick) Pefkos from engaging in the provision of NDIS supports and services to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;
2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service services under a Participant’s Plan
3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and a NDIS Participant.

This banning order is made under section 73ZN(2)(a)(i) of the NDIS Act and takes effect on 3 April 2025 at 5:00 pm (AEDT). The order remains in force permanently, across all States and Territories of Australia.

On 19 March 2025 a banning order was made under 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting BEVERLEY Faith Rita DUFFIN from:

1. Being engaged, or otherwise employed, in any role where she is solely responsible for the provision of any NDIS supports or services to NDIS participants; and 

2. Having unsupervised contact with NDIS participants as part of the provision of NDIS supports or services. 

In this instance: 

• Contact includes physical contact, face-to-face contact, oral communication, written communication and electronic communication; and
• Supervision means direct and in-person observation. 

The banning order is effective from 05:00pm ACDT on 28 March 2025 for a period of four (4) years.

On 18 March 2025 a conditional banning order was made under Section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013, this order permits and restricts Lisa Robson from the following activities:

Permitted Activities:

1. May provide the following supports only to their existing clientele:

• Daily personal activities as defined in the NDIS Support Catalogue;

• Community access and social participation activities;

• Non-clinical daily living support.

Prohibited Activities:

2. Must not:

• Provide any high intensity daily personal activities as defined in Schedule 2 of the National Disability Insurance Scheme (Provider Registration and Practice Standards) Rules 2018;

• Engage in any clinical care activities, including but not limited to medication administration or management;

• Provide care for any clientele with a mealtime management or behaviour support plan in place.

The banning order is effective from 5:00pm AEST on 28 March 2025 for a period of five (5) years and operates across all States and Territories of Australia.

On 20 March 2025 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013. The banning order prohibits Amy Louise Newitt from: 

1.accessing medication intended for NDIS participant/s or administering medication to any NDIS participant/s.

The banning order is effective from 5:00pm (AEDT) on 28 March 2025 for a period of two years.

On 21 March 2025 a banning order was made under subparagraph 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013.

The banning order prohibits Melanie Foley from:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan; and

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan;

The banning order is effective on 28 March 2025 at 05:00pm AEST for a period of 5 years, across all States and Territories of Australia.

On 12 March 2025 a permanent banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 (NDIS Act). The banning order restricts Katerina Formosa from being engaged in the following activities:

1. Appointment as plan nominee of a participant under section 86 of the NDIS Act for anyone other than a person who is an immediate family member.

2. Appointment as correspondence nominee of a participant under section 87 of the NDIS Act for anyone other than a person who is an immediate family member.

The order is effective from 26 March 2025 at 05:00pm AEDT and operates across all States and Territories of Australia. 

On 24 March 2025 a banning order was varied under sections 73ZN(1)(b)(i) and 73ZN(1)(b)(iii) of the National Disability Insurance Scheme Act 2013. The banning order permanently restricting Priscilla James, also known as Priscilla Choi, from being engaged in the following activities:

• Provide or be involved or assist in the provision of any support or service to NDIS participants;
• Provide or perform any supports or services (whether paid or unpaid), for or on behalf of any NDIS Provider which may occasion any personal contact with a NDIS participant;
• Being at or residing at a property funded under a NDIS participants plan. 

This variation to the banning order will take effect from 5:00pm AEST on 24 March 2025 across all States and Territories of Australia.

On 20 February 2025, a banning order was made under sections 73ZN(1)(b)(i) and (iii) of the National Disability Insurance Scheme Act 2013.

The banning order prohibits Freedom Care Group Pty Ltd from engaging, directly or indirectly, in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. Engaging in activities of and as a NDIS provider

2. Delivering supports or services to NDIS funded participants

3. engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services

4. engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services

5. engaging in activities in a non-service and non-support delivery role for an NDIS provider, the extent that those activities have a direct effect on the provision of NDIS funded supports or service to people with disability in the NDIS

The Order is permanent effective from 5pm AEDT on 21 March 2025. 

On 7 March 2025 a banning order for 2 years was made under section 73ZN(2)(a)(i) and 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013. The banning order prohibits Ms Sheree Doherty from:

1.Providing disability supports and services to NDIS-funded participants in the National Disability Insurance Scheme

The banning order takes effect on 21 March 2025 at 5:00pm AEDT for a period of 2 years.

On 20 March 2025, a banning order was made under sections 73ZN(2)(a)(i-iv) of the National Disability Insurance Scheme Act 2013 prohibiting Emilley Suzanne Welsh from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. engaging in activities that a member of the key personnel of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;

2. engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

3. engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

4. engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS.

The order is for a period of two (2) years, effective from 5:00pm AEST on 21 March 2025, across all States and Territories of Australia.

On 7 March 2025 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013. The banning order prohibits Mukundbhai Parekh from:

1. Provide, be involved or assist in the provision of, to any NDIS participant, any support or services under a Participant’s plan.

The banning order is effective from 21 March 2025 at 5:00pm AEDT, for a period of 18 months.

On 20 March 2025, a banning order was made under sections 73ZN(2)(a)(i-iv) of the National Disability Insurance Scheme Act 2013 permanently prohibiting Manbir Rooprai from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. engaging in activities that a member of the key personnel of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;

2. engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

3. engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

4. engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS.

The order is permanent, effective from 5:00pm AEST on 21 March 2025, across all States and Territories of Australia.

On 20 March 2025, a banning order was made under sections 73ZN(2)(a)(i-iv) of the National Disability Insurance Scheme Act 2013 prohibiting Charlene Joy Gatt from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme, including but not limited to:

1. engaging in activities that a member of the key personnel of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;

2. engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

3. engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

4. engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS. 

The order is for a period of two (2) years, effective from 5:00pm AEST on 21 March 2025, across all States and Territories of Australia.

On 20 March 2025, a banning order was made under sections 73ZN(2)(a)(i-iv) of the National Disability Insurance Scheme Act 2013 permanently prohibiting Richard John Kennedy from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. engaging in activities that a member of the key personnel of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;

2. engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

3. engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

4. engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS.

The banning order is permanent, effective from 5:00pm AEST on 21 March 2025, across all States and Territories of Australia.

On 18 March 2025 a variation banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 permanently prohibiting Jarnail Singh from being involved in the provision of supports or services to NDIS participants effective from 5:00 pm on 19 March 2025.

On 12 March 2025, a banning order originally issued on 13 September 2023 was varied. The banning order was varied under section 73ZO of the National Disability Insurance Scheme Act 2013, to extend the current banning order expiry for a further eighteen (18) months and takes effect from 5:00pm AEST on 18 March 2025, and will expire on 18 September 2026.

On 25 February 2025 a banning order was made under 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Rebecca Dixon from the following activities: 

1. Providing or being involved or assisting in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;
2. Engaging in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between Rebecca Dixon and a NDIS Participant. 

The banning order is effective from 5:00pm ACDT on 14 March 2025 for a period of four (4) years.

On 6 March 2025, a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 (NDIS Act). The banning order prohibits Shaun Andrew Matheson from:

1) engaging in any activities that would be delivering support and services to NDIS funded participants;

2) engaging in activities that an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;

3) engaging in activities that would be undertaken as a consultant, to the extent that those activities relate to NDIS supports or services;

4) engaging in activities that would be undertaken as an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services; and

5) engaging in activities in a non-service and non-support delivery role as a provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS.

6) Providing or be involved or assisting in the provision, to any NDIS Participant, of any support(s)or service(s) under a Participant’s Plan. 

7) Engaging in any work or providing or performing any support(s) or service(s) (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and an NDIS Participant.

The banning order takes effect on 14 March 2025 at 5:00pm AEST for a period of 3 years, across all States and Territories of Australia.

On 26 February 2025, a banning order was made under subparagraph 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013. 

The banning order permanently prohibits Mark Cranage from:

1.Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan; and

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan. 

The permanent banning order takes effect on 10 March 2025 at 5:00 pm AEST.

On 3 March 2025 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013. The banning order prohibits Robert Milton from:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan;

3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and an NDIS Participant.

The banning order takes effect on 3 March 2025 at 5:00pm AEDT for a period of 5 years, across all States and Territories of Australia.

On 26 February 2025, a banning order issued on 13 December 2024 under sections 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 (NDIS Act), which was varied on 19 December 2024 under section 73ZO of the NDIS Act, has now been varied again under section 100 of the NDIS Act.

The Banning Order prohibits Edwin Kiyeng from being engaged in the following NDIS activities:

1. Engaging in activities from providing supports and services to people with disability in the NDIS, without the presence of another support worker or supervisor on duty.

This Order takes effect on 28 February 2025 at 5:00pm ACST until 20 December 2026 and operates across all States and Territories of Australia.

On 20 February 2025, a banning order was made under section 73ZN(1)(b)(iii) of the National Disability Insurance Scheme Act 2013.

The banning order permanently prohibits/restricts Eithyn William Cannon from engaging in the provision or management of NDIS supports, services or funding to people with disability in the NDIS, including but not limited to undertaking the following activities:

1.engaging in activities that a member of the key personnel of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;

2.engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

3.engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

4. engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or service to people with disability in the NDIS.

This Order takes permanent effect on 28 February 2025 at 5:00pm AEDT and operates across all States and Territories of Australia.

On 21 February 2025 a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013.

The banning order prohibits/restricts Kulkiran-Jeet Singh from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. Delivering supports or services to NDIS funded participants;

2. Engaging in activities that a member of the key personnel of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;

3. Engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

4. Engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services; and/or

5. Engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS. 

The banning order is in place for a period of five (5) years effective from 5:00PM AEDT on 28 February 2025.

On 26 February 2025 a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013.

The banning order prohibits Gai Akuei from:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan;

3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and a NDIS Participant.

The banning order is effective from 5:00pm on 28 February 2025 for a period of two (2) years.

On 24 February 2025, a permanent banning order was made under section 73ZN(2)(a)(i) of the National Disability and Insurance Scheme Act 2013 prohibiting Angelea Wright from being involved in the following activities:

1) engaging in any activities that would be delivering support and services to NDIS funded participants;

2) engaging in activities that an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;

3) engaging in activities that would be undertaken as a consultant, to the extent that those activities relate to NDIS supports or services;

4) engaging in activities that would be undertaken as an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services; and

5) engaging in activities in a non-service and non-support delivery role as a provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS.

The banning order takes effect on 28 February 2025 at 5.00pm AEDT and operates across all States and Territories of Australia.

On 28 February 2025 a temporary banning order was made under section 73ZN(1)(b)(iv) of the National Disability Insurance Scheme Act 2013.

The banning order Prohibits Barbi-Jade Aubrey from: 

1. providing supports or services to participants funded by the National Disability Insurance Scheme (NDIS), including plan management and support coordination. 

The banning order is effective from 5:00 pm AEST on 28 February 2025 for a period of six months.

On 18 February 2024 a banning order was made under section 73ZN(2)(b) of the National Disability Insurance Scheme Act 2013 prohibiting Mr Mohammad Edelbi , from engaging in the following activities:

1. Creating invoices (regardless of the basis on which the person is engaged by the provider); or

2. the lodgement of invoices for any kind of claims for or other payments of any kind on behalf of any other person; or

3. the lodgement of any kind of claim for, or the calculation of entitlement to, or other financial benefit or payment of any kind on behalf of any other person; or

4. access to any kind of funds or financial instruments used by the provider to make payments for goods or services of any kind; or

5. access of any kind to funds or financial instruments held by or for the benefit of any person to whom the provider provides supports and services.

This order takes effect on 24 February 2025 at 5:00 pm for a period of 5 years and operates across all States and Territories of Australia.

On 18 February 2025, a banning order was made under section 73ZN(2)(a)(i) the National Disability Insurance Scheme Act 2013, permanently prohibiting Chloe Hart from using any of the restrictive practices as described and listed by the Victorian Senior Practitioners Office (VSPO) as being prohibited.

These prohibited restrictive practices include the following as per the directions of the VSPO:

(a) the use of prone restraint (subduing a person by forcing them into a face-down position)

(b) the use of supine restraint (subduing a person by forcing them into a face-up position)

(c) pin downs (subduing a person by holding down their limbs or any part of the body, such as their arms or legs)

(d) basket holds (subduing a person by wrapping your arm/s around their upper and or lower body)

(e) takedown techniques (subduing a person by forcing them to free-fall to the floor or by forcing them to fall to the floor with support)

(f) any physical restraint that has the purpose or effect of restraining or inhibiting a person’s respiratory or digestive function

(g) any physical restraint that has the effect of pushing the person’s head forward onto their chest

(h) any physical restraint that has the purpose or effect of compelling a person’s compliance through the infliction of pain, hyperextension of joints, or by applying pressure to the chest or joints 

This order takes effect from 5pm on the 21 February AEDST 2025. 

On 18 February 2025, a banning order was made under section 73ZN(2)(a)(i) the National Disability Insurance Scheme Act 2013, permanently prohibiting Robert Curley from using any of the restrictive practices as described and listed by the Victorian Senior Practitioners Office (VSPO) as being prohibited.

These prohibited restrictive practices include the following as per the directions of the VSPO:

1. Engaging in activities of and as a NDIS provider

2. Delivering supports or services to NDIS funded participants

3. Engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services

4. Engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services

5.Engaging in activities in a non-service and non-support delivery role for an NDIS provider, the extent that those activities have a direct effect on the provision of NDIS funded supports or service to people with disability in the NDIS

The Order is for a period of three (3) years, effective from 5pm AEDT on 21 February 2025. 

On 17 February 2025 a Five (5) year banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013. 

The banning order prohibits Nathan Avery from being involved either directly or indirectly, in the provision of NDIS supports or services to people with a disability.

The order is effective from 5pm AEDT on 21 February 2025.

On 5 February 2025, a banning order was made under sections 73ZN(1)(b)(i-iii) of the National Disability Insurance Scheme Act 2013 prohibiting The Australian Health Company Pty Ltd from engaging, directly or indirectly, in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. Engaging in activities of and as a NDIS provider

2. Delivering supports or services to NDIS funded participants

3. Engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services

4. Engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services

5. Engaging in activities in a non-service and non-support delivery role for an NDIS provider, the extent that those activities have a direct effect on the provision of NDIS funded supports or service to people with disability in the NDIS

The Order is for a period of three (3) years, effective from 5pm AEDT on 21 February 2025. 

On 5 February 2025 a banning order was made under sections 73ZN(2)(a)(i-iii) of the National Disability Insurance Scheme Act 2013 prohibiting Mr Sanjay Parasher from engaging, directly or indirectly, in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. Engaging in activities that a member of the key personnel of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;

2. Engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

3. Engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

4. Engaging in activities in a non-service and non-support delivery role for an NDIS provider, the extent that those activities have a direct effect on the provision of NDIS funded supports or service to people with disability in the NDIS.  

The Order is for a period of three (3) years, effective from 5pm AEDT on 21 February 2025.

On 18 February 2025, a banning order was made under section 73ZN(2)(a)(i) the National Disability Insurance Scheme Act 2013, permanently prohibiting Kaleb Bloomer from using any of the restrictive practices as described and listed by the Victorian Senior Practitioners Office (VSPO) as being prohibited.

These prohibited restrictive practices include the following as per the directions of the VSPO:

1. being a paid employee of a NDIS provider

2. doing unpaid work for a NDIS provider such as volunteer work

3. being a NDIS provider 

This order takes effect on 21 February 2025 for a period of ten (10) years.

On 18 February 2025, a banning order was made under section 73ZN(2)(a)(i) the National Disability Insurance Scheme Act 2013, permanently prohibiting Joel Sherwood from using any of the restrictive practices as described and listed by the Victorian Senior Practitioners Office (VSPO) as being prohibited.

These prohibited restrictive practices include the following as per the directions of the VSPO:

(a) the use of prone restraint (subduing a person by forcing them into a face-down position)

(b) the use of supine restraint (subduing a person by forcing them into a face-up position)

(c) pin downs (subduing a person by holding down their limbs or any part of the body, such as their arms or legs)

(d) basket holds (subduing a person by wrapping your arm/s around their upper and or lower body)

On 30 January 2025 a banning order was made under 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 restricting Bernadette Elsie Cole from being involved in personally or directly providing NDIS supports or services to NDIS funded participants, which in this instance means:

1. physically touching a person with disability or
2. having contact with multiple NDIS funded participants as part of the direct delivery of a specialist disability support or service or
3. building a rapport with NDIS funded participants as an ordinary part of the performance of normal duties whether face-to-face, or via oral communication, written communication, or electronic communication. 

The banning order is effective from 5:00pm ACDT on 21 February 2025 for a period of four (4) years.

On 5 February 2025, a banning order was made under section 73ZN(1)(b)(iii) of the National Disability Insurance Scheme Act 2013 permanently prohibiting Adam James Pearce from engaging in the provision or management of NDIS supports, services or funding to people with disability in the NDIS, including but not limited to undertaking the following activities:

1. engaging in activities that a member of the key personnel of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;

2. engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

3. engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

4.  engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or service to people with disability in the NDIS.

This Order takes permanent effect on 14 February 2025 at 5:00pm AEDT and operates across all States and Territories of Australia.

On 07 February 2025 a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 prohibiting Mr Jyothikumar Madhavan from being engaged in the provision of any and all NDIS supports or services to people with disability.

This order takes effect on 11 February 2025 at 05:00pm AEDT for a period of two (2) years.

On 7 February 2025 a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013. 

The banning order prohibits Sadat Khosru from:

1) Provide, be involved in, or assist in any way the provision of supports to any NDIS participant or services under a participant’s plan. 

The banning order is effective from 5:00 pm AEDT on 11 February 2025 for a period of two (2) years.

On 31 January 2025 a permanent banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 restricting Tamara Fox from managing and/or providing supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. Delivering supports or services to NDIS funded participants;

2. Engaging in activities that a member of the key personnel of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;

3. Engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or service;

4. Engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services; and

5. Engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS.

The Order is effective from 5pm AEST on 10 February 2025. 

On 31 January 2025 a banning order was made under section 73ZN(1)(a) of the National Disability Insurance Scheme Act 2013 restricting/prohibiting Diamond Community Care Pty Ltd from being engaged in the following NDIS activities: 

1. Engaging in activities of and as a NDIS provider;
2. Delivering supports or services to NDIS funded participants;
3. Engaging in activities that would be undertaken as a consultant for NDIS providers, to the extent that those activities relate to NDIS supports or service;
4. Engaging in activities that would be undertaken as an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services.

The Order is effective from 5pm AEST on 10 February 2025.

On 6 February 2025, a banning order was made under section 73ZN(2)(a)(i) the National Disability Insurance Scheme Act 2013, permanently prohibiting Andrew Blair from using any of the restrictive practices as described and listed by the Victorian Senior Practitioners Office (VSPO) as being prohibited.

These prohibited restrictive practices include the following as per the directions of the VSPO:

This order takes effect from 5pm AEDST on the 9 February 2025. 

On 30 April 2025, a banning order issued against Jhonna Williams was varied under section 73ZO of the National Disability Insurance Scheme Act 2013 to reduce the period it remains in force to three months from its commencement date.

The original banning order was made on 6 February 2025 under section 73ZN(2)(a)(iii) of the NDIS Act, and took effect from 5:00pm AEST on 8 February 2025. It prohibited Jhonna Williams from:

1. Providing, or being involved in or assisting with the provision of, any support to any NDIS participant under a participant’s plan; and
2. Providing, or being involved in or assisting with the provision of, any service to any NDIS participant under a participant’s plan.

The banning order is effective from 5:00pm AEST on 8 February 2025 until 8 May 2025, across all States and Territories of Australia.

On 4 February 2025, a banning order was made under section 73ZN(2)(a)(i) the National Disability Insurance Scheme Act 2013, permanently prohibiting Bianca Hawkins from using any of the restrictive practices as described and listed by the Victorian Senior Practitioners Office (VSPO) as being prohibited.

These prohibited restrictive practices include the following as per the directions of the VSPO:

(a) the use of prone restraint (subduing a person by forcing them into a face-down position)

(b) the use of supine restraint (subduing a person by forcing them into a face-up position)

(c) pin downs (subduing a person by holding down their limbs or any part of the body, such as their arms or legs)

On 14 January 2025 a banning order for Dhanapalan PONNEN was made, under section 73ZN(1)(b)(i) and section 73ZN(1)(b)(iii) of the National Disability Insurance Scheme Act 2013, restricting/prohibiting Dhanapalan PONNEN from:

1. Engaging in activities that an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services 

2. Engaging in activities that a member of the key personnel of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services; 

3. Engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services; 

4. Engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services; 

5. Engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS. 

This order takes effect on 31 January 2025 at 5:00pm (AEST) for a period of two (2) years and operates across all States and Territories of Australia.

On 16 December 2024, a banning order issued against Ms. Rehana Memon on 18 January 2023 was modified under section 73ZO of the National Disability Insurance Act 2013, extending her current two-year ban to a permanent ban.

The Banning Order prohibits Ms Memon from engaging in the provision of management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. Engaging in activities that a member of the key personnel of a NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services.

2. Engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services.

3. Engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services; and/or

4. Engaging in activities in a non-service and non-support delivery role for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS.

The permanent order takes effect from 5.00pm AEDT on 29 January 2025 and operates across all states and territories of Australia. 

On 24 January 2025, a permanent banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 prohibiting Mr Vijay Kumar from:

1. Providing or being involved or assisting in the provision, to any NDIS participant, of any support or supports under a participant’s plan.

2. Providing or being involved or assisting in the provision, to any NDIS participant, of any service or services under a participant’s plan.

3. Engaging in any work or providing or performing any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and an NDIS Participant. 

The banning order takes effect from 5:00pm AEST on 28 January 2025 and operates across all States and Territories in Australia.

On 24 January 2025, a permanent banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 prohibiting Mrs Sarah Jones from:

1. providing or being involved or assisting in the provision, to any NDIS participant, of any support or supports under a participant’s plan.

2. Providing or being involved or assisting in the provision, to any NDIS participant, of any service or services under a participant’s plan.

3. Engaging in any work or providing or performing any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and an NDIS Participant. 

The banning order takes effect from 5:00pm AEST on 28 January 2025 and operates across all States and Territories in Australia. 

On 13 January 2025, a banning order of John Michael Craig TRACEY was made, under section 73ZN(1)(b)(iii) of the National Disability Insurance Scheme 2013 Act (Cth), permanently prohibiting John Michael Craig TRACEY from:

1. engaging in activities that a member of the key personnel of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services; 

2. engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services; 

3. engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services; 

4. engaging in activities in a non-service and non-support delivery role for an NDIS provider, the extent that those activities have a direct effect on the provision of NDIS funded supports or service to people with disability in the NDIS. 

This order takes permanent effect on 21 January 2025 at 5:00pm (AEST) and operates across all States and Territories of Australia.

On 6 December 2024, a banning order was made under section 73ZN(1)(a) of the National Disability Insurance Scheme Act 2013 prohibiting WW Care Pty Ltd from being engaged in the following NDIS activities, both directly and indirectly, including but not limited to:

• engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS);
• engaging in activities that would be delivering support and services to NDIS funded participants;
• engaging in activities that would be provided by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;
• engaging in activities that would be provided by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services;
• engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS.

This Order takes effect on 17 January 2025 from 5:00pm AEST and operates across all States and Territories of Australia. 

On 16 December 2024, a banning order was made under section 73ZN(1)(b)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Assistive Disability Services Pty Ltd from engaging in the provision or management of NDIS supports, services or funding to people with disability within the National Disability Insurance Scheme (NDIS), including but not limited to: 

1. Engaging in activities that an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services; 

2. Engaging in activities that would be undertaken as a consultant, to the extent that those activities relate to NDIS supports or services;

3. Engaging in activities that would be undertaken as an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services; and

4. Engaging in activities in a non-service and non-support delivery role as a provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS.

This order takes effect on 10 January 2025 at 5:00pm (AEDT) for a period of ten (10) years and operates across all States and Territories of Australia.

On 17 December 2024 a banning order was made under section 73ZN(2)(a)(iii) of the NDIS Act of the National Disability Insurance Scheme Act 2013. The order prohibits Kira Lee Drake: 

1. from being engaged, or otherwise employed, in any role where she is solely responsible for the provision of any NDIS supports or services to NDIS participants;

2. having unsupervised contact with NDIS participants as part of the direct delivery of NDIS supports or services. 

This Order takes effect on 10 January 2025 at 5:00pm (ACDT) for a period of four (4) years and operates across all States and Territories of Australia.

On 7 January 2025, a banning order was issued under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013, restricting Timothy Turnbull from:

1. Providing or personally being involved in the provision, to any NDIS participant, of any support or supports under a participant’s plan; or

2. engaging in any work or providing or performing any supports or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may provide access to an NDIS participant’s sensitive personal or financial information.

This Order takes effect on 7 January 2025 at 9.00am (AEDT) for a period of two (2) years and operates across all States and Territories of Australia.

On 17 December 2024 a banning order was made under section 73ZN(2)(b) of the National Disability Insurance Scheme Act 2013 prohibiting Mr Shein Willi Aufa’i from the following: 

1.Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan and:

2.Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan;

The banning order is in place for a period of one (1) year effective from 1 January 2025.

On 11 December 2024 a permanent banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 permanently prohibiting Benjamin Carter from being involved in the provision of supports or services to NDIS participants.

Specifically, the order restricts/prohibits Mr Benjamin Carter, from being engaged in the following NDIS activities:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan;

3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and a NDIS Participant.

The banning order comes into effect 1 January 2025 from 5:00PM.

On 11 December 2024 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Joshua Sbresni from being involved in the provision of supports or services to NDIS participants.

Specifically, the order restricts/prohibits Mr Joshua Sbresni, from being engaged in the following NDIS activities:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan;

3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and a NDIS Participant. 

The banning order comes into effect 1 January 2025 from 5:00PM for a period of two (2) years. 

On 16 December 2024, a banning order was made under section 73ZN(2(b) of the National Disability Insurance Scheme Act 2013 prohibiting Lynette Peters (Morgan) from being involved in the following NDIS activities: any activities that relate to the direct provision of NDIS funded supports and services to NDIS participants, including support coordination and plan management. 

This Order takes effect on 1 January 2025 at 5:00pm (AEDT) for a period of two (2) years and operates across all States and Territories of Australia.

On 17 December 2024 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Ryan Nugara from being involved in the provision of supports or services to NDIS participants. The order prohibits Mr Ryan Nugara, from being engaged in the following NDIS activities:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan.

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan.

3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and a NDIS Participant. 

This Order takes effect on 1 January 2025 at 5:00pm (AEDT) for a period of two (2) years and operates across all States and Territories of Australia.

On 20 December 2024 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 permanently prohibiting Phi Thi CHAU from providing NDIS funded supports and services.

The order prohibits Phi Thi Chau:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan.

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan.

3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and an NDIS Participant.

This Order takes effect on 21 December 2024 at 5:00pm (ACDT) and operates across all States and Territories of Australia.

On 13 December 2024 a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 prohibiting/restricting Ms Sue Gillies from engaging in the provision of management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. Engaging in activities that a member of the key personnel of a NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services.

2. Engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services.

3. Engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services; and/or

4. Engaging in activities in a non-service and non-support delivery role for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS.

The banning order is in place for a period of five (5) years effective on 20 December 2024 from 5:00 pm (AEDT). 

On 29 November 2024, a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 prohibiting Mr Irish from engaging in the provision or management of NDIS supports, services or funding to people with disability in the NDIS including but not limited to:

1. Engaging in activities that key personnel of a NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services.

2. Engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services.

3. Engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services; and

4. Engaging in activities in a non-service and non-support delivery role for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS.

This Order takes effect on 9 December 2024 at 5:00pm for a period of five (5) years and operates across all States and Territories of Australia.

On the 27 November 2024, a permanent banning order was issued to James Ng’Ang’A Irungu under section 73ZN (2)(a)(iii) of the NDIS Act, restricting him from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS), including but not limited to:

1. Engaging in activities that key personnel of a NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;

2. Engaging in activities that would be undertaken by a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services;

3. Engaging in activities that would be undertaken by an auditor for a NDIS provider, to the extent that those activities relate to NDIS supports or services; and/or

4. Engaging in activities in a non-service and non-support delivery role for a NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS.

The banning order comes into effect 9 December 2024 from 5:00pm (AEDT). 

On 26 November 2024 a banning order was made under section 73ZN(2)(a)(iii) of the NDIS Act] of the National Disability Insurance Scheme Act 2013 prohibiting Cyriaque Nijenahagera from providing NDIS supports or services involving direct contact with NDIS participants for a period of four (4) years effective 6 December 2024 from 5:00pm ACDST. 

On 26 November 2024 a banning order was made under 73ZN(2)(a)(iii) of the NDIS Act of the National Disability Insurance Scheme Act 2013 prohibiting Sharna Louise Burnell from being involved in delivering NDIS supports or services involving direct or indirect contact to NDIS funded participants for a period of four (4) years effective 6 December 2024 from 5:00pm AWST. 

On 2 December 2024, a banning order issued on 6 June 2024 under sections 73ZN(1)(b)(iii) and (iv) of the National Disability Insurance Scheme Act 2013 (NDIS Act), was varied under section 100. The Banning Order restricts/prohibits Mr Jorge Guillen from being engaged in the following NDIS activities:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan.

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan.

3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between Mr Jorge Guillen and an NDIS Participant.

This Order takes effect on 2 December 2024 at 5:00 pm (AEDT) and remains in force permanently and operates across all States and Territories of Australia. 

On 29 November 2024 at 5:00 pm (AEDT), a permanent banning order was made against Jordan Hawkins under section 73ZN(2)(b) of the National Disability Insurance Scheme Act 2013 (NDIS Act), restricting him from:

1. Providing or personally being involved in the provision, to any NDIS participant, of any support or supports under a participant’s plan; or

2. Engaging in any work or providing or performing any supports or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may provide access to an NDIS participant’s sensitive personal or financial information (such as banking details).

This order is permanent and takes effect on 29 November 2024 at 5:00 pm and operates across all States and Territories of Australia.

On 25 November 2024 a banning order was made under section 73ZN(2)(a)(iii) of the NDIS Act of the National Disability Insurance Scheme Act 2013 prohibiting Taleena Catheryne Whiting being involved in any role that provides NDIS supports or services to NDIS funded participants for a period of five (5) years effective 29 November 2024 from 5:00pm. 

On 18 November 2024, a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Nirmeen Noufl from: 

1. engaging in any activities related to the provision of services or supports to NDIS participants.

This order takes effect on 26 November 2024 at 5:00pm for a period of three (3) years and operates across all States and Territories of Australia.

On Tuesday 26 November 2024, a banning order was varied under section 100 of the National Disability Insurance Scheme Act 2013 prohibiting Mr Benjamin Kulman from being engaged in the following NDIS activities for a period of five (5) years:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support(s) or service(s) under a Participant’s Plan.
2. Engage in any work or provide or perform any support(s) or service(s) (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any person contact between you and an NDIS Participant.

This banning order is made under section 73ZN(2)(a)(i) and section 73ZN(2)(b) of the NDIS Act and takes effect on 26 November 2024 from 6:01pm (AEDT). The order remains in force until 12:00am on 12 July 2029, across all States and Territories of Australia.

On 4 November 2024, a permanent banning order was made under section sections 73ZN(2)(a)(ii) and 73ZN(2)(b) of the National Disability Insurance Scheme Act 2013.

The order permanently restricts Ms Aluel Deng Mawiir from being engaged in the management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS) and holding a position of authority, control or responsibility of a NDIS provider including but not limited to;

1. Engaging in activities that key personnel (as defined under section 11A of the NDIS Act) of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;

2. Engaging in activities that would be undertaken as a consultant for a NDIS provider, to the extent that those activities relate to NDIS supports or services;

3. Engaging in activities that would be undertaken as an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

4. Engaging in activities in a non-service and non-support delivery role as a provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS.

This Order takes effect on 22 November 2024 at 5:00pm )AEST) and operates across all States and Territories of Australia.

On 21 November 2024 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Talent Dube from being involved in the provision of supports or services to NDIS participants, specifically:

1. Providing or being involved or assisting in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;
2. Providing or being involved in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan; 
3. Engaging in any work or providing or performing any service or services (where paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and a NDIS Participant.  

Effective from 5:00 pm on 21 November 2024 for a period of two (2) years.

On 18 November 2024 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013.  Specifically prohibiting Mohamed Issak from being engaged in the following NDIS activities for a period of ten (10) years.

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan;

3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and a NDIS Participant.

Effective from 19 November 2024 at 5pm for a period of ten (10) years across all States and Territories of Australia

On 18 November 2024 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013.  Specifically prohibiting Ms Hawolul Issak from being engaged in the following NDIS activities for a period of ten (10) years.

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan;

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan;

3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and a NDIS Participant.

Effective from 19 November 2024 at 5pm for a period of ten (10) years across all States and Territories of Australia

On 4 November 2024 a banning order of Nathan John Steven GRAY was made, under Section 73ZN of the National Disability Insurance Scheme 2013 (Cth), permanently prohibiting Nathan John Steven GRAY from being involved in the provision of NDIS supports and services to people with disability effective from 5:00pm on 15 November 2024. This involves the following activities: 

1. engaging in activities that a member of the key personnel of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;

2. engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

3. engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services;

4. engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS.

On 12 November 2024 a banning order was made under section 73ZN(2)(a)(iii) of the NDIS Act of the National Disability Insurance Scheme Act 2013 prohibiting Joshua Adam Lambert from being involved in delivering NDIS supports or services involving direct contact to NDIS funded participants for a period of four (4) years effective from 15 November 2024. This includes:

1. physically touching a person with disability or 
2. having contact with multiple NDIS funded participants as part of the direct delivery of a specialist disability support or service or 
3. building a rapport with NDIS funded participants as an ordinary part of the performance of normal duties whether face-to-face, or via oral communication, written communication, or electronic communication. 

The banning order would operate across all states and territories of Australia.

On 13 November 2024 a banning order was made under section 73ZN(2)(a)(iii) of the NDIS Act] of the National Disability Insurance Scheme Act 2013 prohibiting Harrison Lemayian Kimeei from being involved in delivering any NDIS supports or services to NDIS funded participants under the age of 18 for a period of four (4) years effective 15 November 2024 from 5:00pm.

On 15 November 2024 a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 restricting Mr Somtochukwu Okonkwo from the following activities related to the provision of NDIS funded supports and services to NDIS participants for a period of two (2) years.

1. Prohibited from providing NDIS funded supports and services to people with disability who are under a Positive Behaviour Support Plan of any kind.

2. Restricted to working between the hours of 6:00am and 6:00pm on any given day and must not work longer than 10-hour shifts.

Effective from 15 November 2024 at 5pm across all States and Territories of Australia.

On 15 November 2024 a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 restricting Mr Nnabuike Johnson Omeh from the following activities related to the provision of NDIS funded supports and services to NDIS participants for a period of two (2) years.

1. Prohibited from providing NDIS funded supports and services to people with disability who are under a Positive Behaviour Support Plan of any kind.

2. Restricted to working between the hours of 6:00am and 6:00pm on any given day and must not work longer than 10-hour shifts.

Effective from 15 November 2024 at 5pm across all States and Territories of Australia

On 13 November 2024 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Mr Kitione Tuiqilai from being involved in the provision of supports or services to NDIS participants effective Wednesday 13 November 2024 from 5:00pm, for a period of 2 years, across all States and Territories of Australia.

On Monday 11 November 2024, the original banning order issued was varied under section 100 of the National Disability Insurance Scheme Act 2013 prohibiting Mr Carmelo Davide Graziano from being engaged in the following NDIS activities:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support(s) or service(s) under a Participant’s Plan.

   Note: Without limiting the operation of this order, this includes prohibiting the management of NDIS plan funds, operating in a consultancy or education capacity to NDIS Providers in respect of their delivery of support(s) or service(s) under a Participant’s Plan.

2. Engage in any work or provide or perform any support(s) or service(s) (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any person contact between you and an NDIS Participant.

This new banning order takes effect on 11 November 2024 at 5:00 pm (ACDT) and remains in force permanently, across all States and Territories of Australia.

On 29 October 2024, a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 prohibiting Metuisela Railumu Donu from engaging in any activities that entail the provision of disability supports and services to NDIS funded participants in the National Disability Insurance Scheme, including but not limited to: 

1. Engaging in activities that a member of the key personnel of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;
2. Engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;
3. Engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports and services; 
4. Engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded support or services to people with a disability in the NDIS. 

The Banning Order is effective from 5.00pm QLD time on 8 November 2024 for a period of five (5) years across all states and territories of Australia. 

On Wednesday 6 November 2024, a banning order variation was made under section 73ZO(1) of the National Disability Insurance Scheme Act 2013 (NDIS Act) prohibiting Anh Quoc Vuong from being involved in the provision of supports and services to NDIS participants for a period of two (2) years, effective from 5:00 pm (ADST) on Thursday 7 November 2024.

On 1 November 2024 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Mr Leauanae Kirisome from being involved in the provision of supports or services to NDIS participants indefinitely effective from 5:00 pm on 1 November 2024. This involves the following activities: 

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan.

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a Participant’s Plan.

3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and an NDIS Participant.

On 18 October 2024 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Claudio Osvaldo Martinez from being engaged in the provision of NDIS supports or services involving direct contact to NDIS participants.

The banning order is effective from05:00pm AEST on 28 October 2024 for a period of four (4) years. 

On 22 October 2024, a formal Banning Order was issued against Nicholas under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 permanently prohibiting him from providing supports or services in any capacity to people with disability. Specifically, Mr Nicholas James Kennedy is prohibited from being engaged in the following NDIS activities:

1. Providing any form of support or services to NDIS participants or people with a disability;
2. Being employed or otherwise engaged by any NDIS provider in any capacity; 
3. Being involved in the provision, management, or administration of any NDIS supports or services; and 
4. Having any contact with NDIS participants in a profession capacity. 

The Banning Order takes effect from 5.00pm AEST on 23 October 2024 and operates across all states and territories of Australia. 

On 10 October 2024, a banning order was made under 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 permanently prohibiting Vannara Kheav from providing supports or services to NDIS funded participants under the age of 18 years old in the National Disability Insurance Scheme effective from 5pm ACDST on 21 October 2024.

On 18 October 2024, a banning order was made under 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Criss Adriel from providing supports or services to NDIS funded participants in the National Disability Insurance Scheme for a period of four (4) years, effective from 5pm on 21 October 2024.

On 14 October 2024 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Trevor Mutch being engaged in the following NDIS activities:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan.

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a participant’s Plan.

3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and a NDIS Participant.

The Banning Order is effective from 5:00 pm on 14 October 2024 for a period of two (2) years across all states and territories of Australia.

On 11 October 2024 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Katrine Hay from being engaged in the following NDIS activities:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan.

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a participant’s Plan.

3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between you and a NDIS Participant.

The Banning Order is effective from 5:00 pm on 11 October 2024 for a period of two (2) years and operates in all states and territories of Australia.

On 9 October 2024, the original banning order issued on 9 July 2024 was varied under section 100 of the NDIS Act to change the specified activities as defined in the banning order restricting/prohibiting Mr Sanjay Ratanpal from being engaged in the following NDIS activities:

1. Provide or be involved or assist in the provision, to any NDIS Participant, of any support or supports under a Participant’s Plan.

2. Provide or be involved or assist in the provision, to any NDIS Participant, of any service or services under a participant’s Plan.

3. Engage in any work or provide or perform any service or services (whether paid or unpaid), for or on behalf of any NDIS Provider, which may occasion any personal contact between Mr Ratanpal and a NDIS Participant.

This Order takes effect on 9 October 2024 at 5:00 pm (ACDT) and remains in force until 5:00pm (ACST) on 23 July 2029 and operates across all states and territories of Australia.

On 1 October 2024 a banning order was made under sections 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 prohibiting Shane YULE from engaging in any activities that entail the provision of disability supports and services to NDIS funded participants in the National Disability Insurance Scheme for a period of two (2) years; effective from 4 October 2024.

On 27 September 2022, a banning order initially in force from 5 October 2022 was varied under Section 73ZO(1) of the NDIS Act.

The variation further prohibits Natalie Jane Nortman from being involved in the provision of NDIS-funded supports or services to NDIS participants.

The banning order variation is for a period of 2 years, effective from 5.00pm AEST on 2 October 2024. 

On 23 September 2024 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Mrs Allyson Coleman from being involved in the provision of supports or services to NDIS participants for a period of 2 years effective from 5:00 pm on 27 September 2024

On 16 September 2024, a banning order was made under sections 73ZN(1)(a) and 73ZN(1)(b)(i) of the National Disability Insurance Scheme Act 2013 prohibiting Mr Raheel Chaudhry, trading as RMC Plan Managers, from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme

(NDIS), including but not limited to:

1. engaging in activities that a member of the key personnel of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services;
2. engaging in activities that would be undertaken by a consultant for an NDIS provider, to the extent that those activities relate to NDIS supports or services;
3. engaging in activities that would be undertaken by an auditor for an NDIS provider, to the extent that those activities relate to NDIS supports or services;
4. engaging in activities in a non-service and non-support delivery role for an NDIS provider, to the extent that those activities have a direct effect on the provision of NDIS funded supports or services to people with disability in the NDIS. 

The banning order is effective for a period of 5 years, taking effect from 5:00pm on 27 September 2024. 

On Monday 23 September 2024 , A banning order was made under section 73ZN(2)(a)(iv) of the National Disability Insurance Scheme Act 2013 (NDIS Act) prohibiting Rahul Chouhan from being involved in the provision of supports and services to NDIS participants for a period of three (3) years, effective from 5:00 pm (SA Time) on Tuesday 24 September 2024.

On 24 September 2024 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Gavin Michael Raye from the provision of NDIS-funded supports or services to any National Disability Insurance Scheme (NDIS) participant for a period of six (6) months effective from 24 September 2024.

On 17 September 2024, a banning order was made under section 73ZN(2)(a)(iv) of the National Disability Insurance Scheme Act 2013 prohibiting Ms. Magda Fawzi Elabd Mikhaiel from engaging in the provision or management of NDIS supports, services or funding to people with disability in the National Disability Insurance Scheme (NDIS) for a period of two (2) years effective from 18 September 2024 at 5:00pm.

On 10 September 2024 a banning order was made under section 73ZN of the National Disability Insurance Scheme Act 2013 prohibiting Mr Scott Silvey from providing disability supports and services, directly or indirectly, to NDIS-funded participants in the National Disability Insurance Scheme permanently effective from 13 September 2024

On 13 September 2024, a banning order initially in force from 7 July 2021 and varied on 8 August 2023 was further varied under Section 73ZO(1) of the NDIS Act. The order permanently prohibits Reily David Paul Middleby from being involved in the provision of NDIS-funded supports and services.

On 12 August 2024 a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 permanently prohibiting and restricting Ms Connie Timothy SIVALY from providing services to people with disability. The order takes effect at 5:00pm on 6 September 2024.

On 6 August 2024 a revocation and banning order was made under sections 73P & 73ZN of the National Disability Insurance Scheme Act 2013 permanently prohibiting and restricting Comfort Care Support Services Pty Ltd from undertaking the following activities:

1. Delivering support and services to NDIS funded participants
2. Operating as an NDIS provider 
3. Operating as a consultant in the NDIS Sector 
4. Operating as an auditor in the NDIS Sector 
5. Providing any non-service and support delivery role for a NDIS provider
 

The order takes effect at 5:00pm on 6 September 2024.

A banning order was made against Adam Morris under section 73ZN(2)(a)(iii) of the NDIS Act, prohibiting him from being involved in the provision of NDIS funded supports or services to people with disability with effect from 23 August 2024 indefinitely.

On 20 August 2024 a banning order was made under sections 73ZN(2)(a)(i) and 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Denise Jayne Clissold, for a period of two (2) years from:

1. Engaging in activities that a member of the key personnel of an NDIS provider would undertake, to the extent that those activities relate to NDIS supports or services.
2. Engaging in activities that include the management or provision of accommodation to people with disability in the NDIS.
3. Engaging in activities that include the submission, modification, or cancellation of payment requests through the NDIA’s provider portal or indirectly via third parties (such as plan managers), in respect of people with a NDIS support plan.

Effective from 5pm on 23 August 2024 across all States and Territories of Australia.

On 19 August 2024, a permanent Banning Order was issued to Protos Care Services (PCS) Pty Ltd, under 73ZN(1(a) of the National Disability Insurance Scheme Act 2013. The banning order permanently prohibits and restricts, Protos Care Services (PCS)
Pty Ltd from being directly or indirectly involved in the provision of National Disability Insurance Scheme (NDIS) supports or services to people with disability in the NDIS. Meaning, they are prohibited from directly providing NDIS funded supports or services to NDIS participants. 

The banning order is effective from 5.00pm AEST on 23 August 2024.

On 19 August 2024, a Banning Order was made under section 73ZN(2)(a)(i) prohibiting Alan MacDonald, from undertaking the following activities for a period of two years:

1. Delivering supports and services to NDIS funded participants
2. Operating as an NDIS provider
3. Operating as a consultant in the NDIS Sector
4. Operating as an auditor in the NDIS Sector
5. Providing any non-service and support delivery role for an NDIS provider.

The period of 2 years commences from 5.00pm AEST on 23 August 2024. 

On 20 August 2024 a banning order was made under section 73ZN(2)(a)(i) of the National Disability Insurance Scheme Act 2013 prohibiting Mr Maxwell Bridges from being involved in the provision of supports and services to people with disability in the National Disability Insurance Scheme with effect from 23 August 2024 for a period of 5 years.

On 20 August 2024 a banning order was made under section 73ZN(2)(a)(iii) of the National Disability Insurance Scheme Act 2013 prohibiting Mr Promise Momah from being involved in the provision of supports or services to NDIS participants for a period of 3 years effective from 5:00 pm on 21 August 2024.

On 29 July 2024 a banning order of Joanne Louise MOBBS was made, under Section 73ZN of the National Disability Insurance Scheme 2013 (Cth), permanently prohibiting Joanne Louise MOBBS from engaging in any activities relating to the provision of supports and services, directly or indirectly to people with disability.

The banning order is effective from 5:00PM 16 August 2024. 

On 5 August 2024 a banning order was made under section 73ZN of the National Disability Insurance Scheme Act 2013 prohibiting Ms Donna Roth from providing disability supports and services, directly or indirectly, to NDIS-funded participants in the National Disability Insurance Scheme for a period of 2 years effective from 9 August 2024.  

On 7 August 2024, an original banning order issued 7 August 2021 was varied. The banning order was varied under section 73ZO of the National Disability Insurance Scheme Act 2013. The order, effective across all States and Territories of Australia, permanently prohibits Monika Paniczko from being involved in the provision of supports and services to people with disability, under section 73ZN(2)(a)(i) and (iii) of the National Disability Insurance Scheme Act 2013. The variation takes effect 5:00pm on 8 August 2024.

On 7 August 2024, a banning order originally issued on 7 August 2021 was varied under section 73ZO of the National Disability Insurance Scheme Act 2013. The order, effective across all States and Territories of Australia, permanently prohibits Anastasia Moutsos from being involved in providing supports and services to people with disability, in accordance with section 73ZN(2)(a)(i) and (iii) of the National Disability Insurance Scheme Act 2013. The variation will take effect at 5:00 pm on 8 August 2024.

On 26 July 2024 a banning order was made under section 73ZN(1)(b)(ii) of the National Disability Insurance Scheme Act 2013 permanently prohibiting and restricting Comfort Care Foundation Inc from undertaking the following activities:

1. Delivering support and services to NDIS funded participants
2. Operating as an NDIS provider 
3. Operating as a consultant in the NDIS Sector 
4. Operating as an auditor in the NDIS Sector 
5. Providing any non-service and support delivery role for a NDIS provider

The order takes effect at 5:00pm on 5 August 2024.
